He reviewed the objectives of the 1982 Safety Advisory Committee (SAC) and discussed the recent incident in which a pipefitter was exposed to CO in the methanol plant. Bob Hammann of the Safety and Housekeeping Action Committee attended as a guest. Leonard Hamilton, assistant fire chief, presented a summary of the recently re vised procedure bulletin P-0124, "Fire Protection and Equipment." Dennis Kos reviewed the new procedure bulletin P-0123, "Bypassing and Testing Protective Devices." OLD BUSINESS Blocked Views for Heavy Equipment Operators o There has been some noticeable improvement in the placement of dumpsters out of the way of street traffic. o The dumpster contractor has been alerted to this problem. We will continue to monitor dumpster placement. Insufficient Machine Shop Door Size o The machine shop door on the north end of the main shops building is not tall enough to allow rig entry. Only the smallest rig can enter, with great difficulty. o An engineering evaluation of cost and justification for widening and heightening this doorway was completed. Due to the infrequent need for rig entry and the anticipated cost of modification, this cannot now be justified. SC 12974 LAM014191 Hourly Safety Commit -2- April 15, 1982 Research Maintenance Unit/Craft Team o In the February meeting the HSC recommended formation of a unit/craft safety promotion team in the research shops area. o This recommendation was reviewed by SAC and approved. Formation of the team is progressing. NEW BUSINESS East Plant Steam Vent Noise o The area to the east of the 16T23 condensate tank vent is so noisy that radios or the fire-spill alarm cannot be heard. o This problem was referred to the styrene unit/craft contact supervisor. The area will be surveyed for noise level. If the test results indicate the need for ear protection, signs will be posted. The "EB Preheat" project which is presently scheduled for completion by year end should eliminate the steam venting-noise problem. Fire and Spill Red Tape o There is concern that non-Monsanto personnel present in the plant may not recognize the hazard sign intended by the use of florescent red tape. This actually happened during a recent office annex building evacuation when a contractor went right through the red tape. Could signs be mounted on the tape "CAUTION--DO NOT ENTER"? o The LP&S Department has been looking into the availability of this tape labeled with "ENTRY REQUIRES SPECIAL PERMISSION." Contractor Knowledge of Plant Rules o Contractors entering the main gate often are observed putting the plant rules cards in their pockets without reading. In an emergency these people need to know the plant rules. o Fred Herzog of plant security indicated that this is a continuing problem. Plant guards have been putting greater emphasis on instruction of nonMonsanto personnel on plant rules. This includes more detailed verbal and written communications as they enter the plant. The SHAC is concerned with non-Monsanto personnel safety and will be looking into this problem by mid-year. Safety Suggestion Book Effectiveness o The.HSC members were asked to come prepared to discuss specific items of concern on safety suggestion log book use and effectiveness. Responses to the question were varied. Concern was expressed that in some areas safety suggestion books are not used to their potential. Often answers are only "will investigate" or "MRO written," and nothing happens. Employees are often reluctant to write in the log books. LAM014192 SC 12975 Hourly Safety Committee -3- April 15, 1982 General agreement was reached that safety suggestion book effectiveness could be improved as follows: --Uniform format (suggestion one side--answer on other) --Timely answers (even if under study periodic feedback on status) --Straightforward answers (even if answer is no) --Include MRO numbers --Sign answers o SHAC has recently completed a survey of unit/craft team effectiveness which included safety suggestion books. Based on those findings and HSC concerns, special presentation to PMSB is planned. This communication will include safety suggestion book guidelines. D. D. Kos P.S.
