PLAINTIFF'S EXHIBIT ( MINUTES OF THE SAFETY AND HOUSEKEEPING ACTION COMMITTEE January 17, 1977 Present: Jim Cavender Arnold Glass Harold Lacy Gene Stacy John Tissue Mike Ward (Visitor) RELIEF VALVE PROGRAM -- Mike Ward Mike has been assigned by Plant Engineering Services to make a comprehensive study to develop a procedure to insure a safe and corrective installation of pressure/vacuum relief devices on equipment. He will be in contact with persons most closely associated with the routine relief valve testing program. All people concerned with this program should contact Mike Ward or forward any information and comments they may have on how the procedures and controls can be improved. He will prepare a plan for developing a proposed control procedure with target dates for completion by February 11, 1977. 1976 INSPECTION REPORT RECOMMENDATIONS A. Central Shop Audit - The central shop safety audit is in progress. The team has had two meetings and a third is scheduled for Friday, January 21. The format of this audit is following the outline of the Biennial Loss Prevention Review covered in P-Bulletin P-0131. B. Laboratory Ventilation Problem - A recommendation of the PT Safety Committee will be issued in memo form this week. C. Overhead Work - Jim Gatewood is charged with the responsibility for recommending the necessary control procedure to provide barricades or other means of restricted access where overhead work is in progress. NITROGEN SYSTEMS A number of notices have been sent out. Changes on the nitrogen hose stations are in progress and the adoption of a plant standard all yellow hoses fitted with Thor-type fittings - is being implemented. Red hoses having the proper fittings for nitrogen services will be banded with green tape and will be used for nitrogen services only until they wear out or can be replaced with the standard yellow hoses. Similarly, green hoses already in service will be banded with a red tape until they can be replaced. 13620 SC LAM014839 (ty SHAC MINUTES - 1/17/77 2- - BULLS EYE SIGHT FLOW INDICATORS A SHAC Action Request for information about "bulls eye" sight flow indicators to insure compliance with plant standards was issued on December 8. Twenty-six replies have been received and work is in progress in changing out substandard bulls eyes throughout the plant. The most difficult area appears to be the lactic acid plant where special considerations are necessary because of special materials of construction used in that unit. A formal engineering study will be requested to define the necessary steps to bring Dept. 33 into compliance. RAIL CROSSING SAFETY -- Gene Stacy One of the recommendations made on the investigation of the accident in which a pick up truck was struck by the railroad switch engine at a crossing was: "to refer to the SHAC considerations of several factors (applicable plant wide) which make for hazardous conditions at railroad intersections in the plant". It was recommended that automatic flashing lights of a distinctive color be installed at railroad/street crossings in the plant. A task force on railroad crossing safety was appointed by SHAC as follows: W. L. Riddick, Chairman, W. T. Anderson, H. H. Treaccar, A. J. Stroud and Mike Hancock will review the corrective action proposed in the accident investigation report and develop specific recommendations for providing for improved railroad crossing safety. GUEST INSPECTIONS Members of the PMSB are scheduled to make guest inspections of the plant during the week of January 17. They have been specifically requested to check on fire and entry permits, nitrogen hoses, and site glass problems. A more careful check on nitrogen hoses and bulls eye compliance will be made in the guest'inspections for April. SHORT SUBJECTS A. Toxicity charts have been printed. Some have been laminated in plastic for use in the shops area. Anyone requiring additional copies of the laminated forms should contact Jo Ann Brooks. B. Cooling Tower Platforms - Corrective action to erect guard rails around the cooling tower platforms are in progress plant wide. All cooling tower owners have reported to Harold Lacy's request for action. SC 13621 LAM014840 l SHAC MINUTES - 1/17/77 -3- C. Gas Spill Protection - Harold Lacy has initiated an Engineering Work Order for a study of the appropriate gas spill protection to be provided for the west plant. D. Emergency Procedure Plan Communications Van - Harold Lacy will request permission from the PMSB to initiate an EWO for a mobile unit to provide for an emergency communication center outside the plant. E. Breathing Unsafe Air - As a result of several memos from nonsmokers, the committee will recommend that steps should be taken to limit smoking privileges where it affects non-smokers SHAC will request the PMSB to be given the responsibility to develop a proposal for the necessary control procedures. (The Personnel Dept, will be represented in any consideration of this matter.) F. New System for Reporting Injuries - The new Monsanto system for reporting injuries was approved on a Corporate basis early December, 1976 and became effective January 1, 1977. A plant wide educational program will be made to inform all Texas City employees about how the system works.
