Hahn Subject: December Maintenance Report SCH-49-89 RFS's Worked in December Total Manhours on RFS's 899 4412 PLAINTIFFS EXHIBIT CEL-688 RFMS1s received in December - 38 RFMS1s completed in December - 42 The total number of RFS's completed in 1988 was 6,687 requiring 34,849 manhours. 513 RFMS' were issued in 1988 plus 35 not completed in 1987. This gave us a total of 548 packages in 1988. We completed 495 packages in 1988. Utilities - New Sensall level indicators were installed in the caustic and acid tanks. A new roof was installed on the boiler house. 4-HAP - Converted IBAP to 4-HAP. This turnaround went very well. We had good coordination with Gary Chaney and Operations which made for a very smooth transition. SASPU - SASPU reactor work is in progress. Some material shortages. Should finish early January. Effluent Treatment Plant - Most punch list items are complete. Hanging fire extinguisher and painting. Drum cleaning facility was completed and turned over to Operations. TFE - Construction completed. Unit is in commissioning. Water runs due to begin in early January. Ibuprofen Carbonylation-We experienced a problem with the 3 AOV's associated with the reactor. The valve bodies are Hastelloy B and-the inner balls are coated with ceramic. We found the coating was coming off of the ball, causing the valve to leak through. These 3 AOV's were removed and replaced with high pressure Autoclave Hastelloy B needle valves which were purchased some years ago and stored away by i "QUALITY AND TEAMWORK" A WINNING COMBINATION 009091 SCH-49-89 -2- January 9, 1989 Ibuprofen Carbonylation contd. - L. W. Hartman. Johnny Vaught and Sal Hahn devised a means of adapting a Wee Willie control valve operator to these valves in order to maintain automatic shutdown integrity. Needless to say, a considerable amount of down time and expense was saved in the installation of these valves. Ibuprofen Purification-Completed all punch list items. Numerous RFS's on tracing, leaks, and insulation. Had some problems with gasket on centrifuge lid. Installed braided Teflon in groove in lid. This held pressure. Made water run. No serious problems. Sumps - Completed all tie-ins to collection vessel and acid waste header. Cleaned and inspected sump 12, 13, 14, and 15. Tied in collection vessel to sumps 15 and put in service. Tied in discharges of sump 12, 13, and 14 to sanitary waste system. There is a large crack in the wall separating sump 15 and the flood water sump. As a temporary fix until the new sump 15 is installed, the flood water pump discharge has been routed over to sump 15 instead of pumping to the drainage ditch. Sump 2, 3, and 5 work is nearing completion. Laboratory & Office - Roof on office portion of B-130 has been completed. The roof work on the laboratory is progressing very well. We had a short term electrical outage in the west end of the 400 series lab due to a grounded motor on the water circulating pump in #2 roof top unit. This caused the ground fault on main breaker to trip. New motor is on order. Safety Quality - No injuries reported in December. Maintenance did not have a lost time injury or OSHA loggable in 1988, only had 2 first aid cases.
