Comments on Statistics--J.A. Glass Upon reviewing the statistics for the year it was clear that we had an increase in all categories and a major plantwide effort would be made to reduce injury rate in 1982. C. Contractor Safety--C.M. McCullough Total plant contractor injury cases was down about 10Z, the recordable rates were about the same, and there were five days away cases in 1981 versus three in 1980. Capital project total injury cases were about half of 1980. There were four recordable cases versus six in 1980 and the days away cases were the same as last year with two cases. Overall the trend is toward fewer injuries and better follow-up in the contractor group as a result of having safety super visors working full time on contractor safety. The contractor safety statistics will be added to these minutes. D. CEP Project Safety--Bruce Rule for B. C. Lancaster CEO construction accident statistics showed a rise in total injury cases, twenty-two versus nineteen recordable cases with the frequency rates generally half of the previous year with more than twice the number of manhours worked. Summary table of the CEO construction accident statistics are included as attachments to this memo. II. REPORT OF THE SAFETY ADVISORY COMMITTEE (SAC)--G.W. Daues A. Plantwide Safety Program LP&S reviewed their final preparations for the "Countdown for Safety Program" to be "kicked off" at the January PMSB. A similar program for contractors was discussed. (The PMSB approved the principle of having such a program. Capital Projects (Crow) will work out the details.) B. Supervisor Training Program Due to the press of work associated with recent incidents, LP&S asked to delay their review of this program until February. C. P-Bulletin Update A review of progress on updating P-Bulletins will be made at the PMSB in January. (The status report was distributed with assignments at the PMSB meeting.) McClellan A. 1981 Safety and Housekeeping Inspection October 13,14, & 15 -- J. A. Glass A status report on the items of the Safety and Housekeeping Inspection was passed out with assignments for various items recom mended by the visiting committee. B. Status of S-T-A-R-T Safety Program--J.J. McClellan The Plant Manager.stated that he felt that the START Program had served its original purpose and the new program for Countdown for Safety would place emphasis on those things that count to reduce injuries within the plant. Future publicity will be focused on the Countdown program. C. Hourly Safety Committee Items--J.J. McClellan At the last meeting there were only four members present. The com mittee needs a "shot in the arm". The new committee members are expected to make recommendations for improving it. They are currently working on a survey of the outgoing Unit/Craft Committeemen on how this committee can be more effective. This is a high prlorty item for the new committee, the Incoming SHAC committee will work with the new committee to develop the 1982 program. D. Safety Showers--New ANSI Standard SHAC has prepared recommendations which cover the specifications for new facilities and has forwarded this to SAC for review. E. Emergency Procedure Plan Test December 1981 The December test report is being prepared. Future plans include a communications only test as that seems to be the area of greatest concern. IV. LOSS REPORTING--R.T. HAMMANN/J.A. GLASS Bob Hammann reported that there are no outstanding loss incident reports. V. 1981 S&PP SURVEY AUGUST 31, THRU SEPTEMBER 4--R.T. HAMMANN/J.A. STATUS OF BREATHING AIR SYSTEM PROJECT--G.T. RYAN The target date for having the breathing air system on stream has been set for early February. VII. PLANT MANAGER'S COMMENTS The Plant Manager pointed out that he was not satisfied that we had turned the corner as far as injury frequency is concerned. January injuries indicate that a more rigorous program will be needed. The main tenance zone organization will be used to set up the Plant Manager's meetings with supervisors. J. Comments on Statistics--J.A. Glass We experienced nine total injuries during November. Two of these injuries were recordable: 1) An employee was involved in an accident off plant while on com pany business. He received a fractured arm, multiple bruises, and loss of several teeth; 2) An operator was in the process of lighting a furnace when a flashback occurred resulting in first and second degree burns to his neck and chest. This was the second burn incident we've had within a month involving the lighting of burners. This is a good time to review departmental operating procedures for lighting burners on gas fired equipment. The number of bum cases in the plant has been increasing. We have an excellent video tape, "Bum Emergency", which can be used to help present a safety meeting on this topic. It should help increase employee safety awareness of the effects of turns, and also demonstrates proper emergency care procedures. C. Contractor Safety--C.M. McCullough Safety statistics for total contractor safety and capital project contractor safety are attached to these minutes. November was a good month with no recordable in plant support, and only one in capital projects. D. CEP Project Safety--B.C. Lancaster Accident statistics for the CED construction group were tabulated and listed as attachments the minutes of this meeting. II. REPORT OF THE SAFETY ADVISORY COMMITTEE (SAC)--G.W. Daues A. INTERLOCK BYPASSING Comments received from the PMSB in November were incorporated into the new ^Bulletin on protective devices. In addition, portions of P-0123 on relief valves have been added. The P-Bulletin is now being circulated for final approval. Target date for issue: Mid January 1982. B. NEW PLANTWIDE SAFETY PROGRAM The LP&S superintendent reviewed a proposal for a program aimed at increasing safety awareness in the first quarter of 1982. SAC was favorably impressed and recommends he review it with the PMSB in December. SC 003294 LAM004308 PMSB Meeting Minutes - December 16, 1981 Page 3 C. MANAGING AND COMMUNICATING SAFETY PROGRAMS - SUPERVISOR TRAINING PROGRAM LP&S briefly described a new program aimed at improving one-on-one communications on safety. SAC requested more detail from LP&S at their January meeting. D. STATUS OF P-BOLLETINS SHAC has studied all P-Bulletlns to determine which need updating. LP&S and SAC reviewed SHAC's findings and assisted in assigning responsibilities for updating. These were distributed and reviewed. Status of revisions will be reviewed at January PMSB. E. CONTROL ROOM CONSTRUCTION - S&PP SURVEY III. Plant Services will explore alternative locations, design, standards, and relocation of existing combustible walls and panels in control houses and recommend appropriate action. REPORT OF THE PLANT SAFETY AND HOUSEKEEPING ACTION COMMITTEE (SHAC)--J.J. McClellan A. 1981 Safety and Housekeeping Inspection October 13,14, & 15--- J. A. Glass Copies of the Inspection Report from chairman Needham Coppedge, has been received and distributed to members of the PMSB. LP&S will send out a list of recommendations with it proposed assignments attached for review at the February meeting. B. Status of S-T-A-R-T Safety Program--J.J- McClellan Slides for the 1st quarter of 1982 had not yet been prepared, but expected them to be ready in January. C. Hourly Safety Committee Item--J.J. McClellan There were no items of concern at the last Hourly Safety Committee meeting. Ed Crow presented a program for the Hourly Committee about Contractor Safety. Doug Metten will present the program at the next meeting. D. Safety Showers A review of the safety shower situation pointed out that the emphasis in the new ANSI Standard was on major equipment revisions. The PMSB expressed its concern that we focus our attention on compliance with the OSHA and corporate standards. LP&S will propose to the SAC appropriate actions in compliance with this request. SC 003295 LAM004309 PMSB Meeting Minutes - December 16, 1981 Page 4 E. EMERGENCY PROCEDURE PLAN TEST The test of the emergency procedure plan was scheduled to follow the PMSB meeting on December 16. (This.test was actually held) IV. LOSS REPORTING--R.T. HAMMANN/J.A. GLASS There were no delinquent loss reports outstanding at this time. V. 1981 S&PP SURVEY AUGUST 31. THRU SEPTEMBER 4--R.T. HAMMANN/J.A. GLASS The final report will be issued in late December (the report was actually issued before the end of the year). VI. STATUS OF BREATHING AIR SYSTEM PROJECT--G.T. RYAN Ryan reported that the project was cleaning up a punch list of items and the unit was not expected to be ready until after the end of the year. One outstanding piece of business was the procedure to be used for the notification of the users in case hydrocarbon levels in the breathing air reached action levels. VII. REVIEW OF INCIDENTS The following Monsanto incidents were reviewed: Date Incident No. 11/26/81 81-48 Description Recordable Injury--While lighting pilot light on heater, man sustained burns to neck, chest and band; 35P (D. Kos) 12/4/81 81-49 No injury--Overhead crane cable failure; Shops Building; failure to check after changing electrical (G. McVay) Contractor Incidents NONE VIII. OTHER ITEMS A. OFFICE SAFETY SLIDES, OFFICE SAFETY COMMITTEE--R.T. HAMMANN The PMSB requests that each time our safety awareness slides are pre pared for plant use that the Office Safety Committee prepare similar slides for use in the office areas. GLASS Reviewed the steps to be taken and the general purpose of the Disaster Drill to be held later that day. IX. PLANT MANAGER'S COMMENTS The Plant Manager sent the message that we should not wait until the January program is launched to try to achieve an improvement in safety performance. Line communications should be used to increase safety awareness throughout the plant. J.
