Trafton Beverly/Parham SHAG / .i PLAINTIFF'S EXHIBIT MON-1953 The 1st Quarter Competitive Safety and Housekeeping Inspection for the Shops area was held on April 10, 1984. Based on previous inspection follow-up, hearing conservation, tool/equipment inspections, personal knowledge of STOP and safety log book usage, safety hazards, and house keeping the ratings are as follows: Carpenters Outstanding Insulators Outstanding Truck Drivers Good Operating Engineers Good Machinists Good laborers Good Electricians Good Instrument Good ( Boilermakers Painters Good Good Pipefitters Satisfactory Sane general Garments are listed below: Safety attitudes and personal knowledge of plant safety rules and procedures seems to be at a high level. Most folks feel positive about STOP, however, they are reporting few, if any, observations. In most cases safety log books are not being used, but safety concerns are being adequately handled verbally or by other means. Shop supervision should reevaluate the need for safety logs for the shops. A safety shower and eyebath checklist program has been instituted since the last inspection with craft foremen checking and documenting each shower and eyebath in his area on a weekly basis. All checklists were found to be readily available and up to date. One enphasis area on this inspection was to check action taken on concerns from the last quarterly inspection. Most of the concerns were corrected, but seme are still needing corrective action. These will be listed on J the individual shop comments. Many electrical switches were found throughout the shop that were painted ( blue, and it was not fully understood why this was done. Further probing indicated that this was done to ccrrply with a LP & S recommendation to highlight all cut off switches. The intent of this highlighting should be reemphasized to all shop personnel. SC 000589 LAM001787 2- - Main Shop - General A hose tagging program for tail hoses on pneumatic tools issued by the tool rocm has still not been implemented. (This was reported on last inspection.) The hole in the asphalt at the tool box storage area (SW comer of shop) has not been repaired yet. (This was reported on last inspection.) The holding area on the east side of the shop is cluttered with mis cellaneous materials, janitor supplies, light bulbs, cords, etc. (Reported on last inspection.) The current hearing conservation program in the Main Shop is for each individual to wear ear protection during excessive noise periods. This committee observed excessive noise levels when a boilermaker was hitting sane steel plate. There was no advanced warning and no ear plugs were worn. This committee feels that the current approach is not adequate, and that shop supervision must deal with this problem by defining a hearing conservation program for the shops. There is an inconsistent hose inspection/tagging program. For example, same cylinders have taped hoses and other such as gas welding cylinders do not. Further definition is needed. There was unauthorized storage of items in most comers, tops of cabinets, in I-beam webs, under stairs, etc. Generally, the housekeeping in the Main Shop has deteriorated since the last inspection. The heater roan in the upstairs lunch roan was cluttered with trash, wood, plastic string being used as a pipe hanger from electrical conduit, and a piece of steel pipe was leaning in the back comer of the rocm. Lunches were observed in the work areas throughout the Main Shop even though ventilated cabinets are provided for storing lunches. (Reported last inspection.) Housekeeping in the Training Room was good and orderly even though it was very crowded. Supervision may want to look at reducing the number of tools and equipment for such a small rocm. For example, do they need two grinders? Operating Engineers Considerable improvement in housekeeping since last inspection. i Personal knowledge of plant safety rules, and safety attitudes at a high level. SC 000590 LAM001788 -3- Bulletin board labelled "Safety" had many non-safety related postings. (Reported last inspection.) Fire extinguisher box #11 was empty. This is located SW of building near the steam header. Rig inspection/PM program seems to be adequate. No negative camnents reported from operators. Pipefitters Personal safety knowledge and awareness is good, although housekeeping is-belcw par. There were many tripping hazards throughout the pipe shop. Mainly air hoses and extension cords. Supervision should consider alternate ways of supplying power such as overhead cord reels. (Reported last inspection.) The pipe storage rack on the west side of the pipe shop was messy and several short pieces of pipe were racked in such a way that they would fall if the piece next to it were moved. In the area just outside the Foreman's office, an out of date tag was found on the hose located in the overhead reel. Also, an out of date tag was found on a hose going to an argon bottle. There were several lunches found in the work areas throughout the pipe shop. Boilermakers Safety awareness and commitment was at a high level among the boilermakers, and housekeeping was fair. There were empty coke cans, paper cups, and miscellaneous trash on the floor right in front of the work bench. There was an air hose on the east side of the shop with no utility sign and an out of date inspection tag. There was a ladder #TR-3 with no inspection sticker. Painters Safety attitude and personal knowledge of plant procedures was good, but the inspection was a surprise to them. Housekeeping in the paint shop was fair. There was sane general clutter and cans stored around their work bench. SC 000591 LAM001789 4- - There were four ladders stored behind the water fall air cleaner with out of date tags. Machinists Valve Shop Housekeeping in the valve shop was very good. No deficiencies noted. Machine Shop Safety awareness and personal safety knowledge was good, and house keeping was good. There was some trash outside of the dumpster just north of the building, and there was oil and shavings on seme of the floors in the shop. The safety shower/eye bath checklist was visible and up to date. Garage Housekeeping in the garage has greatly irtproved since the last inspection. The hose storage area on the east side of the garage is still there and still messy. Mast of the hoses appear to have been there for a long time and probably should be discarded. (.Reported on last inspection.) A protective cabinet was built to go around the battery charger system. This looks very good. A steamerette hose with an out of date inspection tag was found on the west side of the building. Carpenters Safety awareness and attitude was very positive, and the housekeeping was excellent. The safety log book has not been used recently, but everyone felt that their safety concerns were being handled pronptly and efficiently. Insulators The safety log book had 6 to 8 entries since the last inspection, and it was evident that their safety concerns received prompt action. Housekeeping in the AW shop and the asbestos change room was excellent. There were two unpinned hoses at a utility station just south of the shop. SC 000592 LAM001790 5- - Laborers The laborer shack was cluttered quite a bit with items being stored on top of cabinets. Seme rearranging of the items being stored there is needed. One of the leadmen expressed some positive carments about the STOP training that he received. Truck Drivers There seems to be a lot of confusion as to what they are supposed to do during an EPP situation. With these people driving vehicles through out the plant, this could be a serious problem, and it needs to be corrected promptly. Electricians Safety attitude and awareness in the electric shop was good. Housekeeping was excellent inside the building, but there was seme clutter outside on the west side of the building. There were lights and extension cords laying on the ground and there were seme pallets ( blocking access to one side of the generator. The safety sign on the grinder on the west side of the shop needs to be permanently mounted. Observed a man working in the test lab without wearing safety glasses. Are they required in that area? No apparent inspection program for slings and cables that are stored in the electric shop. The Electrical Department has started a tagging procedure for plug in electrical cords throughout the shops. It is a colored tag that is coordinated with the hose tag colors. This new program is good, but it needs to be comnunicated better. Instrumentmen Safety awareness and attitudes in the instrument shop were at a high level, but the housekeeping had deteriorated since the last inspection. Access to the safety shower and eyebath in the north half of the shop in the SE comer was partially blocked due to the portable carbon analyzer. SC 000593 LAMOO 7 791 6- - The "No Smoking" sign on the lab hood on the east wall is almost out of sight and needs to be moved to a more visible location. The portable building outside the shop should be repaired or removed. (Reported last inspection.) There are a lot of old instruments stored south of the shop next to the road which should be discarded or stored in a more suitable location. There were several instruments on the floor in walkways and doorways near the south door. Please reply by memo to R. L. Hanmond by May 15, with copies to SHAC. For the Inspection Team J. D.
