Section I - INTRODUCTION PURPOSE OF THE INSPECTION EPA Region 6 Inspector Kerri McCabe (lead), Uma Lad, and Tom McKay (for Spill Prevention Control and Countermeasure (SPCC) inspection) arrived at the LBOC, LLC / Reynolds Lease Tank Battery ("facility"), located from Highway 7 North, thence Arkansas Highway 335, thence N Texas Rd, thence Hayes City Rd, thence Ouachita County Rd 68, thence Ouachita County Rd 511, off oil field lease road at 33.389937, 92.645310; at 1015 on 01/15/2026 for an announced inspection. Mr. Scott Carter represented the facility at the time of the inspection. The inspection was conducted by the authority of the Clean Water Act (CWA) to determine compliance with Environmental Protection Agency (EPA) regulations. There were no other federal, state, or local agency representatives present at the time of the inspection. This facility was last inspected by Inspector Kerri McCabe on 04/03/2024 under the Brine Program. FACILITY DESCRIPTION EPA conducted an inspection on the LBOC, LLC / Reynolds Lease Tank Battery, which contains one (1) oilwater separator, four (4) storage tanks, one (1) saltwater disposal (SWD) injection well, and one (1) active crude oil production well.

Section II - OBSERVATIONS I observed the facility signage, tank battery, pipes and valves, storage tanks, and separators/treaters; see Appendix 1 (Photographs Log), see Photos No. 1 and No. 2. For additional GPS location and photo metadata, see Appendix 1. I observed the loading line for the tank battery with a catchment bucket, see Photo No. 3. I observed the drainpipe for the tank battery with a valve and cap, see Photo No. 4. I observed a sample port with an open-top catchment bucket inside the tank battery, see Photo No. 5. I observed active leaks of crude oil and saltwater inside secondary containment, see Photos No. 5 through No. 8. Section III - AREAS OF CONCERN EPA Region 6 inspectors Kerri McCabe (lead), Uma Lad, and Tom McKay held a closing conference with Mr. Scott Carter with LBOC, LLC after the inspection. The area of concern under the SPCC rule at the time of the inspection was: Active oil and saltwater leaks at valves/couplings inside the tank battery, 40 CFR 112.9(b)(1) (Photos
No. 5 through No. 8). Section IV - FOLLOW UP Inspector will conduct follow-up phone calls with the company representative(s) over the coming weeks and months to ensure areas of concern are properly addressed.
