As recommended, the minutes of the Committee meetings will be posted on the bulletin boards. 2. The new caged ladder at the gas recovery fractionation unit will be in stalled in an area free of the electric cable trays. 3. The Gaskin contractor employees will be instructed to adhere to established work permit procedures. 4. Proper dress and required safety equipment, i.e., hard hats, shirt, shoes, etc., is specified by supervisors depending on the nature of the job. 5All automotive equipment should be checked daily by the driver. Any defective equipment must be taken to the garage and a work order made out indicating the mechanical problem. 6, As recommended, a safety suggestion form, similar to those used by Operations, will be Issued for the benefit of all employees. 7. As recommended, voluntary physical examinations will be given to those employees who are not presently receiving required physicals. The examination will include a medical history, physician examination, urinalysis, blood count, several blood chemistries, chest x-ray, electrocardiograph, breathing capacity, audiogram, eye pressure and visual acuity. The Medical Department will furnish copies of all examinations and labora tory tests to employees' private physician on request. It is not intended that this program be a substitute for an employee's personal physician. LAM 030109 ABS-028035 HEALTH & SAFETY COMMITTEE MEETING 2 Further directives or examination schedules will be issued as required to keep employees informed. 8. As requested, a ladder and platform has been installed for access and operation of the valves in the piperack northwest of tank, J-315. 9. Caution signs have been installed in the crane cabs specifying the minimum operating distance near power lines. The departmental work permit has the same information as a checkoff on the revised form. 10. The following addresses the suggestions made under item No. 4, under "New Business" in the August Committee Meeting minutes: a. A job order has been issued to install an exhaust stack in the PXU analyzer building. b. Two of the PXU pump bleeders have been provided with closed piping to better drain xylenes, etc. c. The GC4 machine in the laboratory room does not pose a problem due to the gases used in the instruments. d. More frequent testing of xylene vapors will be conducted to pin point any problem areas. e. The fire extinguishers have been shelved as requested. f. Platforms for the valving on V-1259, V-1243, and V-1257 are being estimated. g.' The 200 lb. steam valve without a platform is used only when the plot edge valve does not function properly. If necessary to close ^ the valve, an aerial lift can be utilized. h. The electric lockout procedure will aid in reducing the possibility of inadvertent closing of the electric starters at the PXU. Special training for the operation of the breakers and posted breaker operation procedures should be sufficient to insure personnel safety. i. The leaking pumps have been repaired at the PXU. Other necessary repairs are taken care of as required. The reported leaks around P-4425 and P-4427 are well below the TLV for xylene. j. The gauge glasses on the steam drum are blocked off. Engineering Services will study the problem and make recommendations. k. The HDU-1, leaks will be repaired around V-462. LAM 030110 ABS-028036 HEALTH & SAFETY COMMITTEE MEETING 3 11. Engineering Services has inspected the mol sieve fuel gas piping. The present line will be blasted and painted. 12. The tank water draw procedures in Dispatching Department tank farm should be limited to two tanks at a time in a given area. 13. The carpenter shop dust removal system should be revised to eliminate the dust. 14. New operators should be given thorough training in hooking up chlorine cylinders at the cooling water towers. A respirator should be readily available in case a leak should develop. 15. The no-through traffic signs should be installed on the north side of the west district zone building. 16. It was requested that the OP-II traffic light be operated more efficiently for traffic flow from the refinery at 4:00 p.m. 17. The main gate lockers continue to be dusty due to the air distribution system. NEW BUSINESS 1. August safety statistics: 254 reported first aid cases, 13 doctor cases and no disabling injuries. 2. Cardiopulmonary resuscitation training for selected supervisors is scheduled to begin October 1, 1974. Subsequently, the training will be given to selected crafts, etc. 2r. The August first aid cases were discussed as to the cause of the injury. It was expressed that eye protection requirements should be enforced. 4. It was requested that a work permit be issued when unloading material in the HL&P easement. Further, the light company should be notified, and short boom equipment be used. 5. A distinctive letterhead was requested for the minutes of the Health & Safety Committee minutes. 6. Basic safety rules should be given to new contractor employees before they are put on the job. A digest of the basic rules, printed in English and Spanish, was suggested. 7. New restroom facilities were requested for alkylation, C.P. stabilizer, Dubbs 9 and the old flashers. lam 030111 ABS-028037 HEALTH & SAFETY COMMITTEE MEETING 4 8. The marine docks under water piping was discussed. The completed repairs were related and the proposed dock modernization was discussed. 9. Concern was expressed regarding the large diameter slag being used in operating areas and the railroad beds. It is difficult to walk on the slag without turning the ankles. 10. The HDU-1 furnace tube failure fire was discussed. The inspection and record keeping procedures in Engineering Services was related. Higher chromium content tubes are to be installed due to the change in the crude oil properties. 11. The following requests were reported to have been made by the railroad crews: a. The waist high grass and weeds should be removed between the No. 3 and 4 track at the west end of B yard. b. The standing water at the B yard and the "house" track breeds mosquitoes. The drainage should be corrected. c. The new "house" track slag or ballast is too large. 12. The driver of trucks heading to Center Street from power plant No. 2 cannot see trains or engines approaching from the east. A parabolic mirror might be considered. 13. High grass and weeds should be removed in the east property tank farm. 14. A request was made to stop employees from using the automatic lid closing trash cans for a spittoon. IV The noise from the main shop air conditioner was reported to be annoying to the tool room personnel. Acoustical curtains should be considered for noise abatement. 16. Concern was expressed regarding an employee who required medical attention at hydroprocessing on September 21, 1974. The ambulance was reported to be late in arriving and the professional competence of the nurse was questioned. 17. A strong HjS odor was reported to occur at the west door of the DU-1 control room when the wind blows from the southwest. 18. The electric shop motor testing box was reported as having a mal functioning switch. 19. A request was made to renew the piperack signs. (Note: The new signs are complete. Installation is forthcoming). LAM 030112 ABS-028038 HEALTH & SAFETY COMMITTEE MEETING 5 .20 It was reported that the insulation removal at the CCU for the shutdown was not conducted in such a manner so as to keep insulation from falling through the expanded grating. .21 Alamask was again requested to be stocked in the tool room. .22 The flasher Petrochem furnace top platform grating was reported to be loose. 23. It was requested that a single aerial lift be used for sandblasting and painting, thereby improving the overall performance and appearance of the other units. 24. A request was made to repair the speedometers in the company trucks. 25. It was requested that the TECH personnel be instructed to observe the refinery traffic rules. 26. It was requested that the nurses give employees a copy of the recorded diagnosis and the prescribed treatment for first aid cases. 27. The contract employee injury in the platformer No. 5 reactor was discussed. 28. A question was raised concerning the requirements relating to the use of life jackets by the dockmen at the marine docks. 29. The inspection of the power pole cross members should be expedited on the road south of alky. 30. Hot insulation melts the plastic bags used for the disposal of asbestos materials in the field. P- Excessive ammonia fumes were reported around the York unit at alky. 32. Lead burning and the welding of alloys and painted piping should be done on the benches provided with exhausters. 33. A status report was requested on the following alky-C.P. stabilizer items: a. A vent for SO^ removal in the control room laboratory. b. Platform on the York unit vertical reboilers. c. Fumes (NH^ and acid) from the reactor sewers. d. C.P. stabilizer problems, apparently due to the vent lines being too small on the compressors. LAM 030113 ABS-028039 HEALTH & SAFETY COMMITTEE MEETING 6 34. The Safeway scaffolding should be installed so the steps are continuous; at times the sections are reversed. 35. The revised departmental work permit was discussed. The safety re quirement check-offs are documented in the refinery Safety Manual. 36. The old non-adjustable head band firing hood should be replaced at platformer No. 2 and HDU-1. 37. A cart or carrier was requested for the furnace firing Prestolite bottles. 38. Fire watch/welder watch instructions and duties were discussed as relating to a job done at the DHT. 39. A request was made to provide Aromatics Departments solution to use for the treatment of eyes exposed to aromatic hydrocarbon splashes, etc. 40. A request was made for acceptable hot work practices on 650 lb. steam systems. 41. A request was made as to the status of the relocation, etc., of the west zone shop grinder. 42. A platform should be installed at No. 1 outfall for safe access to the pH probes which require frequent maintenance. 43. Ants were reported to be bad in the change house. (Note: Stores was requested to have the exterminator take care of the problem). The following recommendations were made by various members of the Committee: ^9-1 Contractors working on the west side should utilize the west contractor parking lots for reducing the cross refinery traffic flow. The same consideration should include the east side. 9-2 Install remote pump drive operators for the DA unit propane pumps due to seal problems on the existing equipment. 9-3 An Engineering Services study should be made for better packing or the safe removal of fumes from sour gas compressor bleeds. The CCU compressors in particular. safety. 9-4 Qualified operators should be used for welder watch/fire watch for added 9-5 The overhead electric lines at the acid vat should be installed in underground conduit due to the movement of heavy equipment in the area. LAM 030114 ABS-028040 HEALTH & SAFETY COMMITTEE MEETING 7 Prior to the business meeting, the Committee inspected the Lube A, HVI-LVI units. The following items were noted for consideration: SAFETY ITEMS 1. Several of the steam trap lines need insulation for personal protection near the analyzer shelter on the west end of the unit. Also the steam tracing in the west pumpshed. 2. The ammonia cylinders are inadequately secured and the valve shutoff key is missing. 3. The old deteriorated scaffolding should be removed before the scheduled shutdown. 4. The steam lance is missing on the first landing of the HVl column. 5. Many of the column platforms have loose fittings, conduit, etc., which presents a tripping hazard, 6. The steam lance is missing near the top of the LVI column stairs. 7. The steam tracing should be insulated on the pump in the west pumpshed. 8. The rubber hose west of the analyzer building has a crow-foot connection on the control valve, 9. The portable platform ladder on the west side should be junked. 10, The east and west plot edge valves and lines should be identified. 11., Several of the tags are missing on the LVI furnace steam manifold. "12. Remove the rubber hose to the sewer from P-1203. 13. The 3/8 inch oil drain is a tripping hazard on the pump in the west pumpshed. 14. The hose needs a rack at the northwest side of the pumpshed. 15. The level glasses should have protective covers on the cooling tower chemical tanks. 16. The large diameter slag on the west end of the unit is unsafe for walking, LAM 030115 ABS-028041 HEALTH & SAFETY COMMITTEE MEETING 8 17. The noise level plot plan should be replaced in the control room. 18. The surface drain covers should be cleaned. 19. The steel cable should be removed from the base of the HVI furnace stack. 20. Replace the steam hose on the second landing of the HVI column. 21. The platform guard chains should be inspected due to excessive corrosion. 22. Insulate the 1/2 inch steam line west of E-809 and at the east end of the pumpshed on the pipe support. One of the lines should be braced east of the pumpshed. 23. Insulate the 1/2 inch steam trap line at the west side of the cooler box. 24. A safety shower should be installed near the LVI ammonia cylinders. 25. The old steel brackets should be removed on the pipe column southeast of the cooler box. 26. The fire blankets should be covered in the control room. 27. The steam lance is uninsulated on the sixth platform on the LVI column. 28. The pump water spray nozzles should be tested. 29. Steam lance missing under the LVI stripper column. 30. ' Need a bracket or shelf for the extinguisher behind the HVI panel board. 31. Check the sour odor at the east control room door, around P-1221 and the pumpsheds. 32. Safety training should include the precautions to take when working around the cooling water tower - chlorine, sulfuric acid, etc. 33. Valve wrench missing on the southeast turret nozzle. 34. Check the odor around the chlorinator. ELECTRTCAL 1. The units light panel switches should be identified. LAM 030116 2. The switch conduit is loose on D-3430. 3. Ground D-1323 and inspect the ground wire on D-1326 at CWT No. 8. ABS-028042 HEALTH & SAFETY COMMITTEE MEETING 9 4. The open wiring should be removed north of D-1455. 5. The cover is off an electric box on the wall in CUT No. 8 control room. 6. Remove the aluminum ladder in the CWT No. 8 control room. 7. Several conduit covers are off in the west pumpshed. 8. The HVI panel box wiring should be put back in the box. 9. The cover is off the light west of E-765. HOUSEKEEPING 1. Clean out the electric room. 2. Oil on concrete slab around P-1212, 1218 and P-1226. 3. The control room storage room should be cleaned and shelves provided for maintenance material. 4. The assorted debris should be removed from the inside of the column skirts. 5. Clean off the work bench in the LVI pumpshed and discard the scrap piping, etc. 6. The first platform on the LVI column is oily and dirty. 7. Loose insulation above E-748 and also on the platform above and east of E-751. ^8. Oil on second platform of V-671. 9. The Hallikainen viscometer should be cleaned. LEAKS AND SPILLS 1. Steam leak near the sixth or seventh landing of the LVI column. 2. Steam valve packing leak at the west end of the LVI pumpshed. 3. Steam line leak above E-767 and on the platform above E-744. 4. The tracing steam line is leaking at the platform north of E-752. 5. Valve bonnet leak in 3/4 inch line at the LVI-200 distillate manifold near the stripper column. LAM 030117 ABS-028043 HEALTH & SAFETY COMMITTEE MEETING 10 6. Two steam valves leaking on the first platform of the HVI column. 7. Steam tracing leak at the east plot edge block valve manifold. 8. Water is standing on the walkway on the east side of the furnace and also the sour water sump. 9. Valve and flange leak on E-1478 and the head on E-766. MISCELLANEOUS 1. The 6 inch line is vibrating near the top of the HVI column and extending to near grade level. 2. Inspect the loose grating on the third, fourth, seventh, and eighth landings of the LVI column prior to the shutdown. 3. The tugger hoist and cables should be inspected. 4. The control room trash cans should have covers. 5. Identify the utility station piping. 6. The piping should be inspected on the ammonia storage vessel near the field toilet. 7. Repairs should be made in the field toilet - toilet seat, plumbing, doors, windows, sink, etc. 8. The 1 inch steam trap line needs bracing at the westplot edge. 9.- The old chemicaldrums should be removed at the cooling tower. 10. Repair the west control room door handle. 11. Repair the broken door glass at the cooling water tower. 12. The telephone box should have a cover at the cooling tower. 13. Additional shelving should be installed behind HVI panel board. 14. The HVI sour water steam syphon stack releases the unit structures, etc. which goes into 15. The control room doors should have seals to preclude the entry of vapors and also the drain and vent in the control room. LAM 030118 ABS-028044 HEALTH & SAFETY COMMITTEE MEETING 11 16. The walkway on the northeast side of the unit should be repaired. 17. The unit shutdown letters should always have an impact statement addressing the materials which might be encountered by the maintenance people. Pre-shutdown meetings for the crafts should also be continued. 18. The acid injection point at the cooling water tower should have a guard near the motor valve. 19. Inspect the gloves and mat used in the cooling water tower electric room. The hot sticks should be clean and dry. .20 The new scaffolding resting on column handrails should be inspected for strength. .21 The spring loaded pipe hangers should be inspected. .22 Renew the hanger on the electric room danger sign. 23. The ladder under the west end pipe rack should have a platform and cage. 24. Fiber glass or wooden ladders should be provided for changing light bulbs, etc. 25. The pumps, vessels, etc., should be clearly marked for identification. 26. Inspect the pump cooling water drain header in the pumpsheds. 27.
