Construction Co. asked about pumping pond resin to V-10 blend tanks. At about 10 a.m., they were told they could and that close communications would be required between them and us. I pointed out Jimmy Tate, Panel Operator, to Lee's employee and told him this is who he would be checking with as to when to start and stop pumping. I also talked with Jimmy Tate and told him of the plan to pump and not to fill blend tanks over 75%. I went to the maintenance scheduling meeting at 12:55. I walked pass the west side of V-10 dryer room on my way from the Vinyl Control Room to the main tenance meeting. At this time there was no evidence of any type of spill at V-10. I got out of the maintenance meeting at about 1:15. Bobby Hawkins, Vinyl Shift Supervisor, met me at the bottom of the stairs at the Maintenance Shop, and handed me two (2) "E" work orders. I took them upstairs to Mike Skinner, Planner^ and also told Charles Davis, I & E Supervisory about them. I then left the Maintenance Shop headed back to the Vinyl Control Room. As I came by the west side of the V-10 dryer room, I saw slurry on the ground where it had apparently come out of the blend tanks. At this time I heard Bobby Hawkins page me. I went to the control room where Bobby met me and explained that V-10 blend tanks had over filled, and the corrective action had been taken to stop the over flow. At this time 1 called Pete Markey*s office. He was not in. I then paged him. Pete called back and I told him of the spill and that some material went into the sewers but I didn't know if any had gotten into the drain sewer. I told him I would notify Dave Mahler concerning the spill. This I did, but he had already seen it and was checking the sewers. I also called Gil Morgan, Contract Supervisor and told him of problem. I requested him to get Lee Construction to clean up the mess. To Prevent Re-occurrence 1 - Re-calibTate blend tank level tecks. 2 - Have a person wear fresh air line respirator and monitor level in these tanks when pumping pond resin. R. A.
