Comments on Statistics With the closing of the calendar year 1979, we have completed three (3) years utilizing OSHA guidelines for reporting injuries* Our 1979 sta tistics were lower in every injury severity classification showing a significant improvement in safety performance. Six (6) Total Injuries ware reported during December making the year 1979 Total Injuries 19S. Both figures are lowest achieved for a single month or year in the history of this location. We experienced no recordable injuries this month and our yearly total for 1979 was 19, another record low for this location. At the close of December, we completed 271 days without a Lost Workday Case, and only two (2) during the year 1979, one (1) of which was a Days-Away-from-Work Case, all are record lows for this location. As we begin the new decade and the year 1980, even greater challenges will be faced to meet the new local and corporate goals. Our efforts must, as always, be to recognize the need and fulfill the commitment for individual responsibility to complete every job safely. C. Units and Groups with Trendlines Worthy of Comment Dept. 33 (Lactic Acid) had a perfect record in 1979 without a single injury. Depts. 44, 45, 46 have gone eleven months without an injury. The last injury was on January 31, 1979. Depts. 48, 56 turned in the best performance in the last five years, well below the best previous year. Depts. 19, 25, 29 was much better than the best previous year. Dept. 53 was equal to the best previous year. Those groups that have recorded the best year yet were as follows: Carpenters, Shop, Pipefitters, Truckers-Laborers, Stores, Insulators, all Operators, all Maintenance, Depts. 50 and 51. Those departments showing an Improvement over last month Include: Power 2, Boilermakers and Electricians. The seriousness of these injuries as indicated by the recor dable rate was higher than last, but less than half the 1977 rate. The results indicate that there will be a significant effort required to make an improvement in the safety performance of all contractors. This will be part of the overall program for 1980. A plot of the safety performance for total contractors will be attached to the minutes of this meeting. II. REPORT OF THE SAFETY ADVISORY COMMITTEE (SAC)--G.A. Danner for Metten SAC has committed to complete work on the following projects prior to ending their formal 1979 responsibilities: 1. Reporting Level for Loss Incident Reports - Concepts approved by November PMSB have been Implemented in the Accounting Department. D. Metten will revise P-0111 (target 1/31/80). 2. Eye Bath Policy - SAC has been reviewing two concerns identified in the annual inspection; a) use of alarms and green identification lights, b) use of fixed type eye baths rather than the hose type. SAC has requested SHAC survey the field to determine pertinent infor mation on current departmental use of alarms, green identification lights, types of eye baths, etc. Target the survey data back by 1/31/80 with SAC recommendation to the February PMSB. 3. Safety Training Draft - SAC has agreed to meet with SHAC to resolve some conceptual points identified in our initial review of the Safety Training draft procedure. Target 1/31/80. 4. Plant Footwear Program - SAC is reviewing the plant policy on footwear protection in light of the recommendation made in the Annual Inspection Report. Target final recommendation to Feb. PMSB. 5. Color Identification - SAC's major recommedations were accepted in Oct. *79 PMSB. Have to wrap-up some loose ends concerning use of yellow 003409 LAM004423 PMSB Minutes - January 16, 1980 Page 4 alert painting and yellow safety tape. Target to review plant stan dards for the use of yellow alert painting by 1/31/80 with recommen dations to the Feb. PMSB. 6. Longterm Use of MIECA Training - SAC has delayed review of this item until the fall training was completed. We will review longterm desira bility of the use of MIECA training with a recommendation targeted for the Feb. PMSB. SAC feels the following projects should be resolved by the new appointed SAC: 1. Car Seals - Annual Inspection Comment 2. Identification of Hazardous Chemical Handling Areas - Annual Inspection Comment 3. Face Shields - Annual Inspection Comment 4. Work Permits - Annual Inspection Comment III. REPORT OF THE SAFETY AND HOUSEKEEPING ACTION COMMITTEE (SHAC)--Hernandez The report of the work of the SHAC Committee for the year 1979 was distri buted to the board and Raul highlighted those items that are currently still under way. The Plant Manager pointed out that he wanted an actual emergency drill early In 1980. IV.STATUS OF ACCELERATED VESSEL INSPECTION (AVIP)(SHAC)--Hernandez The status of the AVIP program was reviewed in which the target dates on some tanks were being missed for a variety of reasons. Each of these was scheduled to be reviewed In detail prior to setting a new taget date. Members of the board expressed concern over the over-centralization of the paperwork/data handling and verification required to keep this program accurate and up-to-date. They also expressed concern about whether or not the inefficiency in the total system was overburdening the first-line supervisor in keeping it correct and up-to-date. A follow-up report on the status of the AVIP program will be made at the next meeting of the board. V. STATUS OF THE S-T-A-R-T SAFETY PROGRAM--Hernandez The first session of the new S-T-A-R-T program will be held on Tuesday, January 22, 1980. SC 003410 LAM004424 PMSB Minutes - January 16, 1980 Page 5 VI. 1979 ANNUAL SAFETY AND HOUSEKEEPING INSPECTION A status report on the 1979 Inspection Committee recommendation Is being developed and Is scheduled for distribution the week of January 28. The recommendations made by the Inspection Committee are on the agenda for the new SHAC Committee. The Plant Manager requested that the new committee Invite the chairmen of last year's Inspection Committee back for a visit and report on the status of his recommendations. VII. STATUS OF THE S&PP SITE SURVEY RECOMMEDATIONS--J.A. Glass The present status of these recommendations Is being summarized and pre pared for distribution. Target date was the week of January 28, 1980. Recipients of this status report were asked to Indicate the changes. If any, and the status so a new report could be prepared prior to the arrival of Kurt W. Frank who will make the next Inspection the week of February 25. VIII. LOSS REPORTING--R.T. Hammann/J.A. Glass Loss reporting Improved significantly throughout the year of 1980 and at year-end only those most recent events remained unreported. This was limited to three events. IX. INCIDENT REPORT RECOMMENDATION FOLLOW-UP PROCEDURES--Status By PMSB The Plant Manager reported that the members of the board had given him positive feedback on their method of reviewing the Incident Report Recom mendations for their areas. The Plant Manager plans to review the status of each of these recommendations during the mid-year goals review on an individual basis with each of the general superintendents. X. LOSS CONTROL RECOMMENDATIONS--G.L. Tromblee to G.T. Ryan, et al, 8-22-79 Responses of the Individual General Superintendents to these Loss Control recommendations are being received by the Plant Manager and Loss Prevention and Safety. A summary of the comments will be prepared by Loss Prevention and Safety during the first quarter of 1980. XI. PLANT MANAGER'S COMMENTS The Plant Manager pointed out that there were some warning flags showing up to indicate that we were having a let-down in safety awareness in January. Already we have had two recordable incidents and one Day's-Away-from-Work Case. He requested the members of the board to take extra care and pre caution to Insure that a let-down did not, in fact, continue. A bulletin board notice to this affect was being prepared for posting.
