Trafton The plant manager dispensed with the prepared agenda and sent the Board members into the plant to talk with people and bring back their comments, concerns, and observations. The following is an abbreviated listing of items discussed during this meeting. Listing of Concerns by PMSB Members Brubaker - Visited Shops Area - Pipefitter expressed concerns about lack of uniformity of application of beard rules. - Machinists not too aware of the safety suggestion log book - Electricians' log book contained a 1976 question not answered "Should consider incentives for safety" - Hot weather is a contributing factor Newsom - PT - Safety orientation Injury report circulation Ice machine problem (ice used in iced tea?) Improper plugs on heaters and fans __ ,, 5sC 14120 Office and Clerical - Most people feel that present problem is a "field" problem. Ryan - West Plant - Feel like it's a "let down" after startup of AN-5 - Usual problem with goggles and glasses of contract workers - Pipefitter concerns over use of air masks in AN-5 and use of a back-rup fitter with mask "Stick to basics" - and follow procedures STG 2430965 LAM015909 PMSB Minutes - 8/17/77 Page 2 McCullough - Shops, Instrument Dept., West Plant - Lathe problem in main shop - Instrument shop - good follow-up is the key - AN shop entry was hazardous, disorderly - There was an oil spill in the AN-5 WOB area that had been there three days Scaffold problems everywhere - Hoses in walkways - Scales in PE Packout - problem area - Evidence of daily "Quickie" safety meetings - Difference in levels of understanding by people in same unit or work area Supervision walking by little items Danner - Distribution Areas - Not close supervision of pumper-gauger operations Stuart - Plant Streets and Tracks - Still too much traffic in the plant Trafton - West Plant - Contractor safety violations - Dept. 19 personnel concerned about problems related to fogging of cover goggles Sloppy housekeeping - Maintenance supervision Fuller - East Plant People are concerned that accidents are happening to experienced people Clark Mental lapse is believed to be a key contribution to our loss experience.
