His thumb was pinched between the handwheel of a valve handle extension and a support bracket on the handrail. (Mata) Near-Miss Incident--Dept. 16; Three pipefitters sustained contusions to the arms and legs when the load chain of a chain fall they were using came loose and struck them. The contu sions were minor but the incident raised serious concerns on chain fall usage. (Mata) Recordable Injury--Dept. 14; PT Tech nician was sprayed with hot PEB while sampling 14D4 feed. Sustained 1st and 2nd degree burns to left foot. (Wise) Recordable Injury--Dept. 15/16; Pipe fitter cut his left thumb while cut ting a plastic "ty-rap". The "ty-rap" was securing 1/2 inch O.D.
