Comments oh Statistics Overall, the safety performance for 1980 was excellent. We did not have a recordable injury in December. Already in 1981, we have received the warning signals that there has been a let-down in safety awareness that needs immediate correction. C. Units and Groups with Rising Trendlines Units and groups with rising trendlines are Department 50, Department 51, Power 2, plantwide Operators, Carpenters, Truckers and Laborers. Units and groups having the best year yet are plantwide Maintenance, Central Shop, Pipefitters, Machinists, Painters, Electricians, Boilermakers, Analysts, Departments 13/14 and 15/16. The Instrument Department tied their best previous year. D. Contractor Safety--C.M. McCullough 1 The statistics for both Capital Projects and total plant contractor safety are tabulated, plotted and attached to these minutes. Supplemental contractors used in the zones made a significant contri bution to the overall reduction in injury rates experienced by the combined plant contractor injury frequencies. E. CEP Project Safety--B.C. Lancaster Although the total injury cases for 1980 were 113 compared to 98 for 1979, the CED Construction safety performance showed significant gains in total injury rates, recordable rates, lost workday rates and days-away-from-work rates over the 1979 figures. These statistics are summarized in the attached figures. II. REPORT OF THE SAFETY ADVISORY COMMITTEE (SAC)--G.W. Daues A. EPP Drill Comments from several sources in the plant on the December 17, 1980, EPP Exercise have been reviewed by the SAC chairman. Others are still expected to be submitted. The chairman will collect and summarize the comments for review in February at a meeting planned for this subject. When this is completed, we will communicate the results of the review throughout the plant through the regular line organizations and through the formal safety organizations, including the Unit/Craft Safety Promotion Teams. B. Super Hazardous Materials A proposed P-Procedure on the handling procedures for super hazardous materials has been reviewed by SAC, J.G. Trafton and J.A. Glass. By-passing of Interlocks The subject of by-passing of interlocks was discussed, and it is believed that practices throughout the plant on this subject are non-uniform. The chairman will request, via the General Superintendents, a report on the methods currently used for review. D. Manllfts The HSC has suggested that there is a need for a procedure which speaks to a buddy system when a manlift is used to perform work in a super hazardous chemical area. SAC feels that the rules for a buddy system, when working in areas containing super hazardous chemicals, should be the same whether a manlift is used or not. Additional work will be needed on this item. Specifically, a job safety analysis on overall manlift safety will be conducted. The committee will recon sider this item at it's next meeting. Ladder Inspections The bulletin, P-0114, has been revised by D. E. Kaldenberg and will be circulated for review and approval. III. REPORT OF THE SAFETY AND HOUSEKEEPING ACTION COMMITTEE (SHAC) -- C. M. Cruse/J. J. McClellan A. As a result of a comment from the Hourly Safety Committee, PT has established a New Engineer Safety Orientation program to gain better compliance with plant safety practices regarding checking in and out of control houses and other unit safety rules and procedures. B. The Hourly Safety Committee recommended an inspection of the ground fault interruptors being used by contract personnel within the plant. This item has already received attention and those found defective have been removed from the plant. While discussing this item, it was clearly brought out that the Monsanto Materials and Mechanical Tech nology inspectors were using GFI's when entering vessels outside our plant in vendor shops. C. The Material and Mechanical Technology Department has raised the question regarding inplant vessel inspection where there is an agita tor or other moving parts in the vessel. At the present time, M&MT inspectors do not have their own locks for lockout. These will be provided and they will lock out the equipment prior to entry in the same manner as craftsmen or other groups that enter the vessel. D. A report on the activities of the SHAC Committee in preparing for the Annual Housekeeping Inspection has been prepared and will be passed onto the new committee for developing next year's program. F. 1980 Safety and Housekeeping Inspection--Refer to memo MCI Annual Inspection 1980, from J.A.6. to PMSB and SHAC. Target dates for completion to be established--J.A. Glass will contact members of the PMSB to complete these dates. G. Status of AVIP'--The Accelerated Vessel Inspection Program is 95Z on schedule. Those with tanks remaining a little behind are methanol, acetic and AN, and many of these are scheduled for the first quarter of this year. H. Status of S-T-A-R-T Safety Program--The third subject of the S-T-A-R-T safety program featuring safety regarding the proper use of tools is scheduled to be available for use in the plant beginning Friday, January 23. I. Hourly Safety Committee Items--Remaining items on the agenda include 1) manlift safety and buddy system; 2} ground fault interruptors; 3) bicycle use and traffic problems. 1980 was considered a good year for the Hourly Safety Committee. They feel they gained greater acceptance for their role in the plantwide safety program. They have received a good bit of publicity for their efforts and have participated in the Quarterly Housekeeping Inspection Program. IV. STATUS OF LOSS INCIDENT REPORTS, DECEMBER 1980--R.T. Hammann/J.A. Glass There were no delinquent nor outstanding loss incident reports at the end of December, 1980. V. S&PP FOLLOW-UP--J.A. Glass The status of the follow-up on S&PP items for last year's survey has been sent to St. Louis. VI. STATUS OF THE BREATHING AIR SYSTEM PROJECT--G.T. Ryan This project was scheduled for review with Earl Brasfield on January 29. It is scheduled to become a part of the Capital Projects package to be presented in February of this year. EMERGENCY PROCEDURE PLAN DRILL--SAC/Daues The Emergency Procedure Plan Drill preliminary report has been prepared and may be presented at the Plant Manager's Safety Board meeting in February. IX. PLANT MANAGER'S COMMENTS The Plant Manager pointed' out that although we had had an excellent year in 1980, we had gotten off to a very bad start in 1981, and he reminded all the members to focus on the individual responsibility for safety performance.
