No alarms were evident- No-protection equipment was worn. * 5) Employee was offered Medical Examination, which was denied in light of the fact, that annual physicals will be forthcoming in 6-8 weeks. 6) Employees to be monitored for several docking to in order to determine if this is an isolated incident. Sample started at 1800 hours, terminated at 0600. 2) Place of exposures UCM tank farm. 3) Task: Opening a bleed valve which he thought was under a nitrogen purge, whenhe realized his mistake he decided that it would take to long to get a SCBA or airline respirator. He took care of the problem with out the bebefit of respiratory protection. 4) Employee was offered medical examination, which he declined on light of the fact that annual exam inations will be forthcoming in 6 weeks. 5) Additional stations, with airline respirators, have been established in the area. All personnel were made aware of the changes and a monthly inspection schedule has been instituted. 6) A survey of other critical areas of this plant was made and additional stations have been installed in each area.
