According to Joe Lowe, there were five incidences with fresh air masks not working properly during the February 1979 entry into the Cold Box (Dept. 46). Safety meetings were held by Vernon Mapes after the incidences and Manufacturing was made aware of the problem. LP&S COMMENT: Improper use of mask by not attaching hose to clothing as required resulted in hose coming loose from regulator. As soon as we heard about problem, a meeting was called with all involved and they were re-instructed. We experienced no further problems. Paper coveralls were being worn and no holes were made to attach mask hose to a belt loop or a belt. Users must be trained to be sure that the mask hose is supported at the waist. JAG/gr J A.
