Manion Date: November 11, 1985 Subject: Natural Gas Interruption VIS1A As you are already aware, an interruption of the natural gas supply to the VCM Plant occurred on Novem ber 7, 1985 at 8:10 a.m. This interruption occurred as a result of the elective shutdown of the instrument air system at the Lake Charles Chemical Plant for the purpose of scheduled maintenance work. The loss of instrument air caused the natural gas pressure control ler from Gillis, the Flow Controller from United, and the pressure controller to the VCM Plant to fail to the closed position and thereby interrupt the natural gas supply to the VCM Plant. Upon loss of natural gas, both the primary and standby incinerator units at the VCM Plant shutdown. This resulted in the bypass of vents and a condition of non-compliance as related to the requirements of the Louisiana Emission Standards for Hazardous Air Pollu tants (LESHAP). As part of Vista VCM Plant's commitment to maintain compliance with LESHAP regulations, we must identify steps to prevent similar incidents from occurring in the future. Therefore, by copy of this letter a revision to existing operating procedures and associated training of personnel is requested to prevent similar incidents from occurring in the future. It may be advisable to install a sign at the location of the instrument air compressor system to further increase awareness to LCCP plant operators. Please provide me with a written confirmation upon completion of this request such that it may be included in our Environmental File.
