The meeting dis cussion followed the attached published agenda. Injury and Exposure Trends Initially, there was no discussion on the upward trend in the injury area. AHS stated that the increases in VCM overexposures were the result of not chasing leaks. JF shared that he felt that what he saw on the Honeywell strip charts indicated that we don't do as well on finding and fixing leaks. Going back to injury trends, JF asked if the emphasis on safety was there. PJK shared that his supervisors were more active in the safety area but his performance was worse. They were doing two task audits per month. Also, safety meeting times had changed. There were some goods and some bads involved with this change. The biggest problem was in the area of safety permits. Safety was also making a contribution of taking the lead in the Septem ber meeting. CRS commented that the supervisors were involved but plant employees were not involved. Along with that he stated that the major cost control program and safety could not be done at the same time with the same emphasis. He also felt that safety standards should have improved and not require as much attention. JF then asked if we should do anything different. CRS responded that the current cost program intensity and an intense safety effort would be overburdening. DWH noted that safety meetings in Vinyl had been changed from be ginning of shift to end of shift to provide better permit control. Also, Vinyl is doing JSAs. JF asked if Vista has had an impact. CRS, PJK & DWH all responded positively. With current business conditions and the old module down, they were looking for a layoff. Right now employees are concerned about their livelihoods. Customers and job security are the big issues. RAF said that he was not seeing Vista and job security as being in the forefront for the engineers. JF asked if it was appropriate to have employee meetings. After some discussion, the concensus was yes. He then asked for input by 9/25/84 on meeting topics. He also asked us to keep giving thought to how we could improve awareness. PJK asked why our lead sampling frequency appeared so inconsistent. He read off the number of samples reported for each month since the beginning of the year. The high was 40 and the low was 5. This apparent sampling inconsistency was not due to sampling but to timing of reports back from Ponca City. We have audited sampling frequencies for VCM and lead and found them being done as specified. Sub-Committee Progress - Interlock and Alarm Setpoint: CED will have the incinerator and AMS pots complete by 10/1/84. Review of written checkout procedure will be done by REP, RAF, CWT, and CRS. Other fired equipment (boilers/dryers) will be next. Checkout must test shutdown controls either by actual test or by electrical signal. - Blinding: As noted in agenda. Curtis Lloyd to do drafting with template and lettering machine. - Tools: Progress noted in agenda. Additional discussion comparing what was in place versus requirements. JF asked AHS to define requirements under OSHA to CRS, JWW and JF. JF asked if there were other areas where we needed inspection requirements. Other areas mentioned included conduit seals and emergency alarm testing. - Audits - AHS to copy list to committee members for review and comment. JF commented that the output from this would be a list of audit items with frequency set by policy. Other Topics Health Testing Cost Control - In order to improve cost effectiveness, the plant will be working to determine who intends to waive first. Scheduling will then be based on the reduced base of personnel who plan to take the screen. AHS to write cover letter to remind supervisors about why the change when waivers are requested. Sather September 13, 1984 AGENDA - SAFETY COMMITTEE MEETING TIME TO BE SET in 9/17/84 STAFF MEETING VBIA Injury and Exposure Trends Injury performance trends continued to increase since our last meeting. To date, the plant has experienced 21 first aid and four OSHA recordable injuries. This is broken down by department as follows: First Aid OSHA Vinyl Compound Maintenance Lab Receiving/Warehouse Office 21 22 14 1 10 00 _2 _0_ 21 4 The following is a brief summary of each report: - A mechanic suffered a minor burn to his forearm when he touched a hot line during preparation work for a fluid bed shutdown. The insulation on that line had been re moved as a part of the shutdown preparation work. No investigation was conducted. - A Vinyl Chief Operator skinned his knee on the corner of the old module dump box while attempting to unplug the blend tank dump line. The injury occurred while he was climbing up tp reach the flushwater valve from a two-step ladder. The operator was counselled about using an incorrect ladder and the valve handle was modified to allow valve operation from grade. - A Compound Utility suffered a severe laceration to the palm of his hand while cutting a piece of cardboard for the Line III FCM. After making his cuts, he had to tear the cardboard the rest of the way. He switched the knife from one hand to the other so he could tear the cardboard loose. The blade on his safety knife stuck open and cut the palm of his left hand when he switched hands. He failed to follow proper procedure when he switched the knife to the other hand instead of putting it down. ABD00025389 Distribution 9/13/84 Agenda - Safety Committee Page 2 - A Carpenter-Painter-Insulator was stung by a wasp when he disturbed a nest that was concealed under a section of pipe insulation. The injury was not investigated. - A Security Guard suffered a minor laceration to his left fourth finger while climbing down from a tractor cab after getting some papers from the rider. His ring got caught on the door. The guards have been in structed to get the paperwork while standing on the ground. In the area of chemical exposure, the July and August results are shown below: VCM < PEL > PEL PEL W/0 Resp. July August 90 85 3 8 1 4 The overexposures without respirators occurred to a lead operator who did not leave the control room, a mechanic who was blanking D-300 reactor prior to drilling the condenser, and two old module operators along with a vinyl utility training in the old module who could not isolate a specific cause. There was one lead overexposure during the period that occurred to a line compounder who was wearing his respirator. Sub-Committee Progress - Interlock and Alarm Setpoint: CRS to update in meeting. - B1lnding: CUT has reviewed and comments on checksheets for T-701, 702, 703. He currently is reviewing the new module receiver sheets. Some minor blind point changes are necessary on the outlet lines of the old module receivers before CWT can review. Minimal work has been done on the dryers. All vinyl reactors have been initially reviewed. KSA has sent a second issue of 741-744 reactors for review. The P-1 blow down tank has been written and reviewed initially. Boiler feedwater deaerator is in progress. - Tool Inspection: No progress since last meeting. - Audits: Listing of audit type activities has been issued to JF for review.
