Gentlemen:
In response to a meeting conducted at the office of the Assistant Industrial Commissioner, Nev; York Department of Labe on April 13, 1973, and concerning the above subject, I am enclosing the following information, proposal drav/inge and reouired fees:
1. 1 Copy Ea. - Form IH-212, "Application for Approval of Exhaust System Plan" for the; proposed dust col lection system to arrest airborne dust at the joinr cement bag packer.
2. 3 Copies Ea. - Drawings -1 and a 2 "Dust Collecting System for Bag Packer (Fan System #10) prepared by Mr. E. E. Harvey, P.E.
3. 1 Ea. - Georgia-Pacific check in the amount of $30.00 for the examination of exhaust plans as outlined in item 2.
4. 1 Copy Ea. - Form IH-212, "Application for Approval of Exhaust System Plan" for the installation of vent hoods over the additive bag slitter and limestone weigh scale.
5. 3 Copies Ea. - Drawing #7306-1, "Bag Slitter Hood and Duct (Existing fan system #9) prepared by Mr. E. E. Harvey, P.E.
