April 8, 1955 Ret pXyee (Civilian) Wright; Field Dayton* Ohio Present Illness Began last November with dizzy spells, giddiness, hervousnesa, insomnia at times and fatigue early In afternoon* This became increasingly worse until about three weeks ago when he became ao dizzy he fell while at work - at this time he was dizzy but did not lose consciousness - In general he has been sleeping well but states on?. more than one occasion recently had to get up and f>ace the floor. Notices he is nervous* His appetite is good and there lias been no loss of weight* No headaches - has been be* coming more and more deaf in prS t year* He relates that*about a month had for a day sone vague abdominal pain which promptly dis appeared. No diarrhea, little If any nausea or Indigestion and not constipated* No other signs or symptoms referable to cardiac or respiratory system. Genlto-urinary system negative - no h's tory of venereal disease* No arthritis or neuritis - no muscle pain* Past History Has worked for U.S. Government for past 16 years excepting when with T* W. A* for 2 years as Supt* of same work he is now doing. For past 3-1/2 years has heen doing same work which is setting up motors on block, doing some assembling, and testing motors* Had usual childhood diseases - no serious Injury In past, no other recent Illness - operated sometime ago appendectomy in which scar he now has a post-pperative hernia acquired, he states, on present job during unusual exertion* Exposure to Pb* Only In occasionally washing hands and parts In leaded gasoline (there la a rule that non-leaded gasoline be used for washing parts and hands and this gasoline is available according to foreman (Mr* Busse). He occasionally spills gasoline on hands in installing and removing carburetors* No contact with concentrated fluid and washes gasoline from hands after exposure* Very little exposure to carbon monoxide or other combustion vapors and no other vapors or chemicals In building* Physical Exam* Weight* 205 Height* 5*11-3/4" Age* 1*2 Pulse* 72 Temp* 98*2 Blood Pressure* 112/84 In general he is well developed, well nourished and presents a healthy appearance* Color good - not aoutely sick* Eyes * Pupils equal - reg* react well to L and A * Extra ocular muscles Ok Ears * Both drums appear somewhat thickened but intact - canals clear - Hearing impaired apparently about 25$ hearing In left and Qo% right . i: : Hose: Septum`thick * tubblnatea large thick mucous. Throat Teeth have number fillings hut are fairly good gums ; appear Ok - no lead line tongue in mldllne and no tremor ' Pharynx is injected - tonsils medium sized-left lafger than right and show chronic infection * anterior pillars injected. glands: No adenopathy Heart: Hormal In size - rate and rhytym regular - reserve Ok sounds are of good quality and no m u m u r heard. B.P. 112/8ii- Chest: Expansion is equal and fairly good * clear on P and A* Abdomen: Appendectomy 3car lower right with hernia at lower margin-size of egg easily reduced liver and spleen not felt - no pain or tenderness - no inguinal hernia. Extremities: Essentially normal. Reflexes: K.J.: hyperactive Biceps: hyperactive Triceps: hyperactive Ore:.?:- Abdom: +* Epigastric 4*++ Romberg negative - no ataxia - not tremor of fingers Hrinalysis: negative for alb. and sugar Stipple count - Blood sample obtained for Pb determination. Jar and jug left fox' specimens* Impression; (1) This individual present condition is due to the effect of the constant noise in the testing room. (2) That lead in the gasoline used to wash parts and hands Is insignificant as a causative factor. (3) That carbon monoxide or gasoline vapors are not present in sufficient concentrations to be a causative factor. It has been shown that any interference with the impulses originating in the semi-circular canals as by damage to the auditory nerve may well cause disturbances of equilibrium - this possibly may be temporarily due to nerve fatigue.
