Owens Coming Fiberglas industrial hygiene results of welding fume ex posure of craftsmen while working in the wool basement. Please note if any personal samples were collected on workers who were working inside the D-6 "B" zone forming fan. All other industrial hygiene survey results conducted in the wool basement. 4. University of Wisconsin results identifying the bacterial agent as thermophylic actinomyces and the insuing illness as hypersensitive pneumonitis. 5. The claimant's mcJical file which includes pulmonary function tests, chest x-rays, and any examinations or other tests pertaining to the respiratory sys tem. 6. Written programs on respirator usage and on confined space entry. Dates both programs were implemented. Do you or the employee's foreman know of any operations in which the claimant would be subjected to working in a confined space (including the D-6 "B" zone forming fan)? 7.
