All h is to r ic a l r e v ie w s m ust have a beginning and our sto r y actually b egins in 1 826, o r 1827, when an application was m ad e to the L e g isla tu r e of the State of Maryland for the in co rp o ra tio n of the B a l t i m o r e and O h io R a i l r o a d , w h ic h w a s , tit a s , th e f i r s t c h a r t e r e d and fully organized railroad com pany in the United States. T his w as the fir s t r a ilw a y that w a s c o n s t r u c te d , e ith er in E u rop e o r in_ the United States, for the general conveyance of p a ssen g ers or 1 products between distant points. The first division of the railroad 4 was open for the tra n sp o rta tio n of p a s s e n g e r s in 1830, and ran from B altim ore to Endicott M ills, a distance of approxim ately 15 m ile s; -1 h o r s e s and m u le s fu rn ish ed the traction p o w e r. The fir st C h a irm a n o f the M ed ica l and S u r g ic a l S ectio n o f the organization, that was then known as the A m erican Railway A s s o c ia tion, Dr. D. Z. Dunott, pointed out that with the estab lish m en t of tran sp ortation w a s a ls o e sta b lis h e d an occupational h a z a r d , the resu lts of which m ade n e c e ss a r y , very early', a railw ay su r g ic a l serv ice. He em p h asized this very strikingly by pointing out s t a tistica lly that " p ra ctica lly the p a yrolls of the ra ilro a d s today (that is in 1921) has b een im p a ired or killed within a ten y ea r p eriod. " For exam ple, in the year 1911, 3,602 em ployees w ere killed and 126,039 w ere injured and the total num ber of p erso n s killed in r a il w a y o p e r a t io n w a s 10, 396 and 1 5 0 , 159 w e r e in j u r e d . In 1 9 6 5 , 316 e m p l o y e e s w e r e k i l l e d and 3 0 , 1-10 w e r e in j u r e d , and th e t o t a l n u m ber of p e r so n s k illed in ra ilw a y operations was 6 ,2 6 1 and 4 7 ,0 5 7 w ere injured. T h ese figu res are im p ressiv e even though the total em p lo y m en t on the r a ilr o a d s dropped fr o m 1, 6S9, 809 in 1911 to 6 3 9,934 in 1965. (See attached chart) The reduction in total of all people killed and inj ured in railw ay operation w as a c co m p lish ed in spite of the en o rm o u s in c r e a s e in c r o ssin g accid en ts incident to t h e g r o w in g u s e o f th e a u t o m o b i l e . It ca n c e r t a i n l y b e c l a i m e d th at so m e of this d ram atic im p rovem en t can be attributed d ir e c tly to the work of this Section in establishing physical standards which make p o ssib le an em p lo y e e to sa fely perform his job. However, we should get back to d iscussing first things first, and, thus, I would like to give briefly the historical background of the A s s o c ia t io n o f A m e r i c a n R a ilr o a d s . It i s "the lin e a l d e s c e n d e n t " o f much earlier organizations, such as the T im e Conventions form ed in 1872, and the G en era l T im e Convention which, in 1883, e sta b lish ed the sy s te m o f standard tim e by whi ch the whole N ation now i liv e s , and the Railw'ay A ccounting O fficers .A ssociation, form ed in 1880, which helped develop uniform railroad accounting. The A s sociation of A m erican R ailroads was created by the m erger in 1934 of the A m erican Railw ay A ssociation , A ssociation of Railway E x ecu tiv es, the R ailw ay A ccounting O fficers A sso cia tio n , the Railway- T rea su rers O fficers A sso cia tio n , the Bureau of Railway E conom ics 4 and other o rgan ization s. 10 i The parent A m erican Railway Association apparently began about 1872, when its chiei b u sin ess was the adoption of time sch ed ules for p assen ger trains. The detailed sy stem of standard time was adopted for p ractical p u rp o ses m 1883 and provided definite inform ation resp ectin g the changes required in the schedule of every train on each railroad , so as to p reserv e unbroken the r e la t i v e t i m e in c o n n e c t i o n w ith t r a i n s on o th e r r o a d s . It i s i n t e r e s t i n g in this r e sp e c t to co n sid er the confusion that we have at the p resen t tim e in our standard and daylight saving tim e. A code of uniform train sig n a ls was adopted in N ovem b er of 1884, and the adoption of joint block sign al in terlo ck in g ru les was estab lish ed in 1892. A per d iem plan was begun in 1886 and presen ted a s a code in 1902. A uniform code of dem urrage ru les was established in 1909. The A m erican R ailw ay A sso c ia tio n was re-o r g a n iz e d in 1919 and then included a M edical and Surgical Section, with the following subjects under consideration: 1. Separation of ic e fro m drinking water in equipm ent and in stations. 2. Quarantine Regulations. 3. W ater U s e for D rin k in g and C ulinary P u r p o se s. 4. Placarding Condemned Water Supplies. 5. Sanitary Code. 6. Hospital Standard. 7. T raum atic Hernia. 8. P reven tion and C ontrol of Occupational D is e a s e s and H az ards. M r. R, H. A ishton, P r e sid e n t of the A m erica n R ailw ay A s s o c ia tion, on N ovem ber 6, 1920, appointed a tem porary C om m ittee of D irectio n for the M ed ical and S u rgical Section, w hose duty it was to organize the Section, elect tem porary o fficers, and supervise the Section's a ctivities until such tim e as the Section might for m ally elect its su ccesso r. The first meeting of the Committee of D ir e c t io n w a s h e ld in N ew Y o r k on D e c e m b e r 1, 1920. It e le c t e d o fficers and adopted tentative regulations to govern its activities. A lso, Dr. Albert W inters, C hief Surgeon of the Pittsburgh and Lake E rie Railroad, who is here today, advises me that his father was Chief Surgeon, p reced in g h im fro m 1904 to 1941 and participated in these a c t i v i t i e s . A l s o , th e f a t h e r o f D r . A. H. A n d r e w s , J r . , who i s on our program today, w as a consultant for the Rock Island from 1899 to 1945. Aishton, P resid en t of the A m erica n Railw ay A sso c ia tio n , in his ad d ress at the first m eeting of the .Medical and Surgical Section, stated that an am algam ation of the A sso cia tio n of A m erican Railway Chief Surgeons, the New York and New England Railway Surgeons A ssociation, and the Railway Section of the Southern M edical As- 12 sociation, resu lted in the organization of this Section. The Section on Industrial M ed icin e and S u r g e r y of the Southern Medical A s sociation w as provided for at tne L exington M eeting in 1913. O r ig inally it was Southern States A sso cia tio n of Kailwav Surgeons, actually a S ectio n on R ailw ay S u rg ery . Changed in 192 8 to S ection on R ailw a y S u r g e r y . C hanged in 19-15 to S e ctio n on Industrial M e d ic i n e and S u r g e r y . It w a s n o ted that in 191-1, D r . D u n can E v e was the P resid en t of this Section and in 1916 Dr. Southgate Leigh was the President. A ctually, th ere w as no true am algam ation, although the three groups m entioned, undoubtedly, had an in ter-lock in g m em b ersh ip . It i s a l s o a p p a r e n t th a t th e A s s o c i a t i o n o f R a ilw a y C h i e f S u r g e o n s was, undoubtedly, the p red ecesso r of our group. Unfortunately, b eca u se they did not publish their p r o c e e d in g s , I cannot find out when they w ere organized nor who their original m em bers w ere. Dr. Robert G raham has furnished me with a copy of the by-law s of May 1, 1916, which state: 1. T h e o b ject of th is a s s o c ia tio n is to im p ro v e and p rom ote the uniform ity of m ethods and practice. 2. T he m e m b e r s of the a s s o c ia tio n sh a ll c o n sist of the C hief Surgeons of the R ailroads of A m erica, who are eligible for m em b e r ship when dues are paid. 3. O fficers of the association shall consist of a President, V ice-P resid en t, S e c reta ry -T rea su rer and three Executive m em b ers, who with the P r e sid e n t and V ic e -P r e sid e n t shall constitute the Executive Board. 4. The officers shall be elected annually by ballot. 5. T he P resid en t shall appoint all C om m ittees. 6. The r e g u la r m eetin g sh a ll be held sem i-an n u ally, tim e and place to be determ ined by the Executive Board. 7. T h e annual d u e s s h a l l be $ 5 , 0 0 , p a y a b le May 1. M e m b e r s in arrears two y ea rs shall be dropped from the roll. Dr. G raham , in a p erso n a l com m unication, a d v ise s m e that w hile m e m b e r s of the two groups w ere la r g e ly the sa m e , and func tions, so far as m edical education and d iscu ssio n s, were sim ila r, that the A ssociation of Railway Chief Surgeons rem ained as a sep a r a te e n tity at l e a s t u n til 1 9 3 3 . It i s a l s o a p p a r e n t that th e y h eld sep arate m eetin g s. F or exam p le, an annual m eeting was held in Chicago on October 14, 1924. The provisional program was: 1. T ra u m a tic E p ilep sy , postponed from p reviou s m eeting. 2. Chief Surgeon's responsibility for man-power. 3. G en era l M ed ical R e-ex a m in a tio n of Old E m p lo y ees in the T r a n s p o r t a t io n D e p a r t m e n t ; (a) How it had b e s t b e conducted; (b) B orderlin e c a s e s in which th ere m ay be so m e question as to the physical cap acity of the individual to continue in his regular work; (c) The p rob lem of the p h y sica lly disqualified em ployee. 4. The M eans by which Sanitation of T o ilets in Stations and P rivies on Company property can be regulated by the Chief Surgeons 13 and Sanitary O ffice rs. 5. Infected T o n sils and T eeth and their effect on the p rolon ga tion of disability from injury. To go back again to o r ig in s, one of the groups that certain ly contributed to the m em bership of our organization 'as the New York and New England A sso cia tio n of Railway Surgeons, and Dr. J. R obertson K now les, C hief M edical Consultant to the B oston and Maine R ailroad , has been very helpful in giving m e som e of the background of this organization. T his group was interested, since its org a n iza tio n in 1 830, in the m e d ic a l and su r g ic a l p r o b le m s a r is in g in the ra ilw a y industry, and v e r y m any fam ous p h ysicians and s u r geons were m em bers. Dr. D. Z. Dunott, in his a d d ress at the inaugural m eetin g of the Section in 1921, dem onstrated great foresigh t, understanding, and w isd o m . He m ust have been a trem en d ou s p e r so n and I w onder i f a n y o f th e m e n in o u r p r e s e n t g ro u p r e m e m b e r h im . It is a p parent from h is r e m a r k s, and th ose of the P resid en t of the A m e r ican Railway A ssociation , that the Chief M edical O fficers of the ra ilro a d s in th o se days w ere held in v e r y high e ste e m , a situation which has not alw ays existed during the intervening y e a r s. Som e o f D r . D u n o t t ' s r e c o m m e n d a t i o n s , m a d e at t h i s f i r s t m e e t i n g , 4G y e a r s ago, for the p resen t and future activity of the .Medical and S urgical S ection are worthy of rep ea tin g in view of how c lo s e ly they fit in with our p resen t objectives. I have som e copies of his c o m plete recom m endations and th ese are available for those of you who are interested. Among his recom m endations are: 1. "That w h erev er p o ss ib le , only 'Hospitals rated as c la s s A by the C o lleg e o f Surgeons be r eco g n iz ed , and w here r a ilr o a d s have their own h o sp ita ls, that such institutions not already so cla ssifie d will be brought to such standard. " 2. "in v ie w o f the occu p a tio n a l hazard and s ta tis tic s p r e s e n te d , it would s e e m im p era tiv e that g r e a te r care be given to the m atter of physical fitn ess, not only to the applicant for serv ice, but to the em p lo y ee a lrea d y in s e r v ic e w here such em ployee by p h y sica l or mental failure may cau se serio u s accident to the traveling pub lic or fellow em ployees. " 3. "The head of the m ed ical and su rgical serv ice should report to the Chief of the Operating Departm ent. " 4. "in li n e with xny a p p ea l fo r a c lo s e r c o o p era tio n o f the su r g ic a l s e r v i c e , I would urge a m o re general in terest on the part of the su r g e o n s in the op eratin g p ro b lem s that have to be m et by the railw ay o ffic ia ls. " T h is is pertin en t, in view of the p r e se n t ob jective of this group in m aintaining a very close liaison betw een the m edical se r v ic e and m anagem ent. Dr. R obert Graham furnished m e with a copy of the index o f the p ro ceed in g s o f the Annual M eetings of the M edical and S urgical Section of the A m erican Railway A ssociation for the years 1921 to 192.9. T h is is m o s t i n t e r e s t i n g to r e v i e w and it would b e h elp fu l if we could have a continuing index of all of our m eetin g s, which could be brought u p -to -d a te at the end o f each y e a r . T he n u m ero u s and important su b jects that are outlined in this index illu stra te the activities of the Section over these years. As m ost of you know, this added a great deal to cur m eetin g s, and we are v ery p lea sed to have had the D irector of the C laim s R esea rch Bureau with us at all su b seq uent m eetin gs. At th is m eetin g , we w ere favored by a v e r y in terestin g address by Mr. Owen Clark who w as the Chairman of the Interstate C om m erce C om m ission at that tim e. His speech is worthy of r e view. Although this is not strictly h istorical, I will spend a few minutes here because this is one of the few places where our r e s p o n s ib ilit ie s as C h ief M e d ic a l O ffic e r s a r e sp e lle d out; the o th er p la c e s as you know a re by the Su p rem e Court and the Railroad A d justm ent Board. Mr. C lark pointed out that the Interstate C om m erce C o m m issio n had no ju risd ictio n over the ph ysical qualifications or the type and frequency of exam in ation s for railway em p lo y ees, a l though they did p r e sc r ib e r u le s and regulations in the sam e field for m o to r c a r r i e r s . He s ta te d that the r a ilr o a d s "can and should do the job th e m se lv e s . " He reco m m en d ed against a b ill introduced by Senator Kefauver where the C om m ission would have been directed to p r e sc r ib e m in im u m stan d ard s for the p h ysical qualifications of all railroad op eratin g p e r so n n e l and the frequency of re-ex a m in a tio n , and with appropriate p e n a ltie s. T h is proposed legislation resu lted from s o m e s e r i o u s p a s s e n g e r tra in w r e c k s both in the E a st and in the W est which w e r e a lle g e d ly ca u sed by m a n -p o w er failu re. He stated "You have unquestionably developed excellent standards, p a r ticularly with regard to the kind and frequency of physical exam in a tio n , but th a t i s o n l y p a r t o f th e j o b ............. the fa ct r e m a i n s that s o m e 18 r a i l r o a d s h a v e l a g g e d fa r b e h in d and h a v e n<~` a c c e p t e d vo-jr r e c o m mendations. Unfortunately, there is a disturbing lack of uniformity am ong the d ifferent r a ilr o a d s , as I know you are w ell a w a r e ." He definitely states "at the very le a s t, h ow ever, gentlem en, vour recom m ended standard should apply uniform ly throughout the r a il road industry as an absolute m inim um , and I b eliev e you can m ake a great and lastin g contribution to railroad sa fety , if you redouble your efforts to convince railw'ay m anagem ent that th ese standards m ust be adopted fully and without m odification. " I b elieve this is a real ch allen ge to us and one of our best argum ents for convincing managem ent of the n e c e ssity of the importance of our objectives. The Joint M ed ical-L egal C om m ittee was appointed during 1957, and the fir st C om m ittee co n siste d o f Dr. R. J. Bennett, C hief S ur geon of the Elgin, Joliet & E astern Railway; Dr. Harvey N elson, C h ie f S u r g e o n of th e Soo L ine; and D r . W. K. M i s h l e r , C h ie f S u r geon of the E rie-L ackaw anna Railroad. Representing the legal side w e r e M e s s r s . E w i n g , G e n e r a l C l a i m s o f the San ta Fe; W. F. The first m ajor project of the C om m ittee was to p rom ote the im partial m edical testim ony plan with great su ccess. This Committee has been an im portant part of our organization sin ce that tim e and it has m ade sev era l im portant contributions, one of which was a r e com m ended pre-em ploym ent X -ray program for the railway industry which has been w idely adopted. In 1 9 5 8 , the m e e t in g w a s held in E d g e w a ie r P a r k , M is s is s ip p i. T h e C h a i r m a n w a s D r . I. G o l d o w s k y , M e d i c a l D i r e c t o r o f th e C e n tral Railroad of New Jersey. The Secretary was Mr. F. J, Parker. During the yea r 1958, the A ssociation of A m erican Railroads auth orized an expenditure of $30,000 to inaugurate a study of the e f fects of D ie se l fum es at the U niversity of Pittsburgh, Department o f O c c u p a t io n a l H e a lth , G r a d u a te S c h o o l o f P u b li c H ea lth , arid a D ie s e l F u m e s C om m ittee w as activated. Dr. R. S. A yerle, A s s i s t ant M edical D irector of the Pennsylvania Railroad, was appointed Chairman of the activated C om m ittee. As you are all aware, this study w as taken o v e r by the United S tates Public H eal th S e r v ic e after the fir st y e a r and is continuing at the U n iversity of Pittsburgh. " A lso at that tim e, the AAR advised that they would "maintain a sm all staff composed of three persons, namely, M essrs. " The new Plan of Organization of the Operating-Transportation D iv ision of the A ssociation of A m erican Railroads provided "The M ed ic a l and S u r g ic a l A d v iso r y C o m m itte e should be c o m p o se d o f 15 m e m b e r s a p p o in t e d b y th e G e n e r a l C o m m i t t e e . It s h a l l e x e r c i s e general su p ervision over the m atters of interest to the M edical and S u r g ic a l O f f i c e r s o f M e m b e r R o a d s . Some confusion concerning the length of serv ice of the Chair man and V ic e -C h a ir m a n of the M ed ical and S u rgical A dvisory C o m m ittee developed in 1963, which was r e so lv e d by D r. W illiam E. M ishler resign in g after his one y e a r tenure in office, making Dr. Harvey N elson, Chief Surgeon of the Soo Line, Chairman. The Chairman originated a new C o m m ittee called the "Steering C om m i tte e . " It c o n s i s t e d o f t h r e e m e m b e r s to a s s i s t h im in c a r r y i n g out the duties of his o ffice. T h is has b een continued sin ce that tim e. In 1964, a m e e t in g w a s p la n n ed fo r L a s V e g a s, N evada, which w as anticipated by our m e m b e r s h ip , but, unfortunately, it had to be called off because of the threatened Nation-wide railway strike. Thi m eeting was subsequently arranged to be held in Chicago on Septem b e r 21 and 2 2 , and th is a ls o h ad to b e c a n c e le d . In 1965, a M e m b e r sh ip M e e tin g w a s held in C hicago, Illin ois. D r. H arvey N e ls o n 's title w as chan ged at that tim e from C hief S u r geon to M edical A d visor of the Soo Line R ailroad. Dr. Nelson was C h a irm a n and M r. W. E. N e lso n o p e n e d the m e e t i n g w ith th e f o l l o w i n g s t a t e m e n t "I w ould li k e to c a l l to order the 1964 Annual M eeting now being held in 1965. At this m eeting we w e r e a d d r e sse d by Mr. John A. R isendal, D irector of the Claim s R esearch Bureau of the AAR, who replaced M r. K. A. C a r n e y , on the s u b j e c t o f " M ir a c u lo u s R e c o v e r y and E sto p p el" which continued our d e sir a b le c lo s e association with the Claim s Research Bureau. At this m eeting, the C om m ittee on M edical Standards p resen ted us with a new approach to this p r o b le m in the form of a publication entitled "A Guide for A p p lication of .Medical Standards by O ccu p a tio n a l P r o file . " T his r e p r e s e n te d a tr e m e n d o u s amount of work and coordination of the C om m ittee's report over a period of years and is of great value to all railw ay C h ief M ed ica l O ffice rs. T h is b r i n g s us up to th e p r e s e n t t i m e . It i s m y u n d e r s t a n d in g that at this m eeting, we are being advised that our status is again changed from that of a C om m ittee to a M edical Section. N aturally, this is p leasin g to all of us who h ave b een active in this o r g a n iza tion over the years. I have enjoyed presenting som e of the background of our ex cellen t organization, and, in c lo s in g , I would ask if there are any among the m em bership who have inform ation concerning our origins that has not b een m ade a v a ila b le up to the p r e s e n t tim e , that it should be added to this as an addendum , so that this h isto r ic a l r e view will be more com plete.
