These are the minutes of the February 22, 1984, SHAC committee meeting.
Unit Laboratory Inspection
All unit, laboratory inspections have been completed (see attachment). PMSB should consider annualizing the unit laboratory audits.
Plant Emergency Drill
SHAC will assist in monitoring the plant emergency response drill to be held shortly in AN.
General Permit
New general permits have been obtained. The general permit procedure will be reviewed by the individual PMS8 & SHAC members prior to its introduction in the plant.
SHAC Action Plan
The SHAC action plan will be covered the last SHAC meeting of every month.
Status of Safety Awards
The President's Safety Award flag is flying and there was good support of the flag raising.
The bulletin board notice on the jackets was posted and good response is being obtained. Delivery is still expected to be about April 1.

January/February Safety Syndrome
Two people were burned Monday night in Lactic Acid when a condensate hose slipped out of a tank that was being filled. The two people had small areas burned on their feet or legs. The procedure used was improper.
Fire-spill Alarms 2/11 - ACY spill 2/11 - ACY line water leak 2/21 - HCN tank flange leak (loose flanges)
The HCN flange leak was detected by area analyzers and correction of the flange leak went very smoothly.
1984 SHAC Goals
The goals will be reviewed by Tromblee about March 1.
Hourly Safety Committee
The next HSC committee will be on (the last Tuesday) the 28th at 1:30 p.m. at the Personnel Conference Room.
There is no policy or rule that says that keys must be in the ignition of cars at all times. This will be referred to SAC.
Preliminarily, it has been determined that no permit procedures were violated in the PA blinding incident. It may be concluded that stainless steel blinds will be required in this application.
There is no better equipment for sniffing, but plant safety will continue to look for new improved equipment for sniffing. Sniffing procedures and performance have proved to be excellent.
The SHAC member will be invited to the HSC meetings for the next few months (see attached list).
PMSB
The PMSB will generally be held on the third Wednesday at 9:30 a.m., at the VIP conference room. A SHAC member will be invited to attend each of these meetings for the next few months (see attached list).
Enhancement of the STOP Program
Paul Bailey will investigate enhancement of the STOP program. Areas to be investigated will be trend analysis and wards for turning in STOP cards.

1984 Annual Inspection
The decision as to whether the annual inspection will be a "full blow" inspection or an interchange with Chocolate Bayou has a significant impact on what is done in preparation for the annual inspection. Feedback from G. L. Tromblee will be needed on this decision before sub-committee assignments can be made.
Quarterly Inspections
The 1st Quarterly Inspection of 1984 will be sometime in the first two weeks of April. Emphasis areas will be developed at the next meeting. Replies from the last Quarterly Inspection are due the 28th of February. A proposed schedule will be available by the next meeting.
