Collins on the VCM release of 10/11/75. The chunk in the swirl drain valve on D-400 would not allow the valve to close, allowing some VCM to go into the sewer during charging of D-400. If the swirl valve had been checked (if closed light was not on, it should have been checked at the valve) this release would not have occurred. D. F. Johnson has instructed his people to check the valves in the header under the reactor before starting each charge (swirl drain, dump, chem wash valves closed lights must be lighted in the control room or the valves checked in the field to be sure they are closed). Also, M. C. Jemigan is working on the modicon program to allow charging VCM only if the chem wash, swirl drain and dump valves are closed on that reactor.
