The Maintenance Planning Group is currently preparing AFE's
for the following items (see attached memo):
A) Cumulative AFE bv MD (AFE estimate of S437.000 submitted): 1. H2S monitors - replace (24) monitors ($82,701) 2. Repairs & inspections of all permanent lift devices (23) O/H hoists ($10,423 ) 3. Setup turbine over-speed trip testing program ($46,020) 4. Construct ramps over ground level piping (S29,675 ) 5. Sulfur Loading Rack personnel protection (S 2,6 7 5 ) 6. Repairs to E/P Carpenter Shop - termites (S3,580) 7. Inspect/test wire rope & slings ($21,795) 8. Reset 12" prod, line on supports @ Alky ($21,795)
9. Platforms on Recycle Comps. @ *2 Ref. ($5,990)
10. Install platform on top of E/P Condensate Tk (S3,905) 11. Rpl. temporary pipe supports @ MTBE-BTX-SULF ($37,795) 12. Install adequate lighting < E/P Weld Shop ($7,935) 13. Install adequate lighting @ W'/P Weld Shop ($7,195) 14. Install adequate lighting @ Quintana Weld Shop (S7,935 ) 15. Install adequate lighting @ E/P Machine Shop ($7,935) 16. B) Insulation repairs @ 52-53-55-71 Tanks - XRC submitted AFE to Management (AFE under reuew for justification).
C! Repair leak on ~1 Tank - Tank was externally inspected ana Management is rewewing Indtech's report.

D) Pressurization of Control Rooms - MD is investimating safety requirements and control room air intakes in preparation of writing an AFE.
E) Barricade for Sulfur Pit - Maintenance completed installing walkways and barricades (J.O. #151860).
F) Asbestos removal/sealing - MD has prepared bid package per Operations' lists, Contractors to submit bids 7-15-92 for review and bid tabulation. An AFE will be generated from bids.
G) Testing of emergency generators & fire pumps - "Weaver Industrial" is currently testing this equipment on a routine basis.
H) Inspection holes in Insulation - All future turnaround AFE ' s are to include money to make repairs.
I) Work orders have been written to repair all discrepancies in the Maintenance Shops and Warehouse/Garage areas and repairs are in progress. Maintenance has completed (63) of (87) reported discrepancies (72.4%) - Of the remaining (24) safety discrepancies (9) are included in the $437,000 Cumulative AFE which MD submitted.
Attached is an annotated copy of the (56) page "work list" out
lining the required repairs and comments.

In order to correct the deficiencies listed in the "1992 Safety Audit" we are writing the following AFEs:
Control Room Pressurization - East Flant Control Rooms were originally equiped with pressure fans but have all deteriorated and are not in service. Besides pressurization filtration should be inclded for Alky Unit and South Side (H2S hazard). At Quintana vestibules similar to those on West Plant Contol Room doors should be added to allow existing equipment to meet design specification.
Asbestos Abatement m East Plant - Operation submitted list of locations of exposed asbestos. Scope of work and bid package is near complete we plan to bid the job and then write the AFE.(expect S250M)
Tanks 52, 53, 55, 7 1 Insulation Repair - failing panels cause personnel nazarc. AFE submitted to clamp and repair only $263M.
Cummulative AFE - Michey Harris (BHF) wrote AFE S640M for all Safety Audit Repairs over $2,000. Major portions include; H2S monitors, lifting devices and chokers, replace temporary pipe supports with permanent, piug inspection holes ('although included m AFE those with asbestos will be 95% acne on separate asbestos AFE , .

1. Work orders have been written authorizing work to be performed at each of the unit locations (See attached 1992 SAFETY AUDIT REPAIRS). Work which has been directed to be work coder repairs are logged in as entries to a separate work oraer log
book .

4. A separate AFE is being prepared for Control Pressurization. If emergency work orders are written, the will be performed on controllables as needed.

The Scope of the 1992 Safety Audit Repairs is limited to. the 5.
