Troy Stikkey {.,/ I - Z/ - Z..0 Date 6ENFORM-019-8 (10/31/2019) 1 Section I - INTRODUCTION CVS Pharmacy Inspection Date 12/18/2019 PURPOSE OF THE INSPECTION EPA Region 6 inspectors Lee McMillan and Chuck Ruple arrived at the CVS Pharmacy Store at 11:05 AM on December 18, 2019 for an unannounced retail sale of general use pesticides inspection. We met with Mya Cabrales, Store Manager and Jessica Veloz, Assistant Store Manager. I presented my credentials to Ms. Cabrales and informed her that this was an EPA inspection to review all pesticides available for sale regarding compliance with the Federal Insecticide, Fungicide, and Rodenticide Act (FIFRA). FACILITY DESCRIPTION CVS is a drug store and health-drive company run by CVS Caremark. CVS pharmacies work to provide a local supply for prescription and over-the-counter medications. In addition to offering name brands, CVS and Caremark (the parent company) offer a long list of company brands. The store operates 9AM - 10PM and has a total of 14 employees. Section II - OBSERVATIONS During the inspection, the EPA inspectors reviewed the stocked products sold in the store and took photos (See Appendix 1). The products were as follows: Deep Woods OFF! Products Coleman Insect Repellent Total Home Insect Repellent Natrapel Insect Repellent Lysol Products Clorox Products Formula 409 Multi-Surface Cleaner SC Johnson Fantastik All-Purpose Cleaner All products were properly registered with the EPA, had registration numbers, and labeling were in compliance with FIFRA. Section III - AREAS OF CONCERN EPA Region 6 inspectors Lee McMillan and Chuck Ruple conducted a closing conference at CVS Pharmacy at 11:38 AM on December 18, 2019 for the inspection. I observed no areas of concern at the time of the inspection.
