For the reasons stated in the fallowing paragraph, we charge for this service and request that a deposit of *100.00 accompany each request for a narrative report. This charge is for approximately 20 minutes, the minimum time we feel is needed to respond to such a request. If the physician needs additional time to complete your request we will bill you for the balance due when the report is completed. Upon receipt of the deposit, we will give the chart and your letter to Dr. Eckman and you can expect the report in approximat ely two to three weeks. Because most of our patients do not need narrative report for legal purposes, our charges for consultation, visits and other services do not include the physician7s time and expertise to prepare one. Most of the requests we receive are for information contained in the physician* s patient notes. However, it is time consuming for our physicians to review a patient's records and develop a narrative report answering specific questions. The answer to other questions requires the physician's opinion as an expert. He will consider each request for additional information to which the physician must respond as a new request and will expect a deposit to accompany the request. He will be glad to work with you and prepare the information you request. He must charge for this service because of the amount of physician time and expertise required to proper 1y assemble this information and complete the report.
