MINUTES OF SHAC COMMITTEE MEETING December 10 , 1976 Present: Harold Lacy, Chairman Jim Cavender Gene Stacy Arnold Glass I. 1976 PLANT INSPECTION REPORT RECOMMENDATIONS The following recommendations were developed by the SHAC Committee for dealing with the safety recommendations of the 1976 Plant Inspection Team. A. Audit of the Central Shops SHAC appointed a Task Force with Leonard Schwab as chair man, John Ringer, Jim Ingram, George Orum and Russ Furley as members to study the recommendation of the Inspection Team and to propose an action plan for overcoming the problems outlined. Clyde Hebert will be the SHAC member contact with this Task Force. B. Audit of Protection of Laboratory Atmospheres from Contamination This safety recommendation was assigned to the PT Safety Committee for study and development of an action plan. C. Review of Overhead Construction Specifications The committee assigned this safety recommendation to Jim Gatewood for review of the current practice with a request to report back to the committee on January 10. D. Review of Practices Associated with Vessel Entry and Fire Permit Procedures - The committee plans to use this as a key item in the next quarterly inspection scheduled for mid-January. E. Off-shift Fire Crew Response and Fire Extinguisher Compliance with OSHX Arnold Glass and the LPS Department will provide a response to this safety recommendation. F. Review of Quick-Disconnect Fittings This safety recommendation is covered in a P-Bulletin being printed for distribution before the end of the year. P-BULLETIN STATUS Listed below is the status of the P-Bulletins being prepared for issue. P-0115 "Flammable and Combustible Liquids Storage and Dis pensing Areas" - This bulletin has been printed and is ready for distribution. (Issued week of 10-13-76) P-0117 "Fire Entry and Special Permit Procedure" - This bulletin has been revised, approved and sent out for final signatures. P-0126 "Nitrogen Distribution" - This is currently in Printing and is expected to be ready for distribution early next week. (Issued week of 10-13-76) P-0130 "Respiratory Life Support Systems" - this has been revised, approved and is being circulated for final signature. P-0131 "Biennial Loss Prevention Reviews" - this bulletin has been revised, approved and is ready to be retyped for final signature. Ill. GAS SPILL PROTECTION Phil Lewis described a plan for providing contaminant - free air supply for the Styrene control house and office building. The system provides for an electric motor driven blower to take air from a high level intake, force it through a carbon bed and water scrubber into the intake air conditioning system in both buildings. The committee reviewed the proposal, approved it in principle, and Plant Engineering Services plan to continue to develop the design of the actual installation and preparation for a PAFE to cover cost of the project. SHAC (Harold Lacy) will issue an engineering work order to study what should be done to provide an adequate supply of breathing air in emergency centers in the rest of the plant. ACTION REQUEST -- Bull's Eye Incident Harold Lacy is issuing an action request for all operating areas to review the bull's eye installations in their systems for compliance with plant standards. V. OFFICE SAFETY COMMITTEE A. The Office Safety Committee continues to be active in dealing with matters of interest to that group. They have received good reports on recent safety meetings and plan to send out a holiday safety brochure to all employees' families. B. Evacuation of office buildings needs to be reviewed so that the proper directions for evacuation can be posted on the bulletin boards. At present, there are different systems currently in use in the various offices. This study will be assigned to the Office Safety Committee for resolution. VI. LOST TIME ACCIDENT REPORT RECOMMENDATION Jim Robinson requested that the SHAC committee consider how people throughout the plant might review their areas for unique hazards which might breed complacency and lead to serious injury. This subject will be reviewed at the Plant Manager's Safety Board meeting on December 15. VH. MISCELLANEOUS ITEMS A. Cooling Tower Platforms Harold Lacy is sending an action request to owners of cooling towers to remind them of the importance of providing the adequate personnel protection in this area. Lacy reported that the Department 10 installation on CT-1 is complete. There are four kinds of areas that they found needed attention. 1) Handrails 2) Openings large enough for a man to fall through 3) Concrete covers 4) Tripping hazards These have been corrected on CT-1. It might be of interest to other cooling tower owners to inspect their own area for these hazards. SC 13632 LAM014851 SHAG Minutes - December 10, 1976 Page 4 B. Fire Alarms There are some people reporting that they are unable to hear the fire alarms in their parts of the building. Anyone who wants an improved alarm in their area should see their area supervisor and request an improvement. It is up to the department to obtain the necessary alarm systems that are appropriate for their area. C. Microwave Ovens An inquiry was made regarding the care that must be exercised in the use of microwave ovens in the plant. John Fox has reported that the only requirement is that a notice be placed on the doors leading into an area where a microwave oven is in use. This is to notify people who have pacemakers that they should stay away from an operating oven. Usually the distance is about four feet with the oven door closed or six feet if the oven door is open. People who have pacemakers have already been alerted to this microwave exposure and how it would affect their pacemakers. D. Pedestrian Crosswalk Painting A request has been received to mark the crosswalk northwest and northeast of the Main Office Building as a crosswalk area.
