Bob discussed the Loss Prevention and Safety Department's functions, personnel, and their respective responsibilities. Mark Riddle discussed the revision to P-0123, "Bypassing and Testing Protective Devices", and announced that the Unit/Craft safety training at Danforth Campus would be March 10 and 11. He also announced that the STOP training program for Chiefs/Leadmen was proceeding on schedule and STOP cards would be brought to the next meeting for the HSC members to review. UNFINISHED BUSINESS Cooling Tower Fan Hazard In January the Carpenter representative to the committee stated that a safety hazard existed on all but one of the cooling towers. Even though the fan drive motors are locked-out prior to entering the fan area, the fans turn naturally because of the wind drafts created in the tower. Attempting to stop the heavy fan blades, which sometimes turn at a high speed, is hazardous. CT-8 has brakes on the shaft which can be applied. Although this doesn't give 100% protection, it does give considerably more protection than having nothing. Action: There was considerable discussion on this problem again. The committee was told that it was brought up in the PMSB meeting, and that Plant Engineering (Hammond/McClanahan) was actively pursuing this. Action: There was also much discussion on this topic and the committee was told that this was also considerably discussed at the PMSB meeting. The PMSB has asked SHAC to canvass the plant to see what the different practices/procedures are and then to go to SAC to determine a plantwide policy. LAM013508 SC 12312 Page 2 NEW BUSINESS A number of concerns were surfaced by various members of the committee. After discussing the individual concerns it was decided that most should be taken to the appropriate unit/craft safety committee for actions. There were two concerns, however, that can impact the entire plant. They are discussed below: 1. Extension Ladders An extension ladder was found put up backwards in Zone 1--much discussion about correct installation of ladders followed. Proposed Action: A memo will be written to supervision requesting emphasis in this area. 2. Barricade Tape A concern was voiced about barricade tape--tape is being left up after need no longer exists. Therefore, many times tape is ignored. Proposed Action: A memo will be written to supervision requesting emphasis in this area. Chairman's Comments The Chairman asked that a copy of these minutes be posted in the control rooms and given wider spread distribution so that people will know that their concerns are being brought to the HSC's attention, and also to aid in communication. Proposed Action: This request will be discussed further at the next meeting.
