Personnel dosimeter monitoring testing conducted again on March 11, 1983 indicate you were exposed to a vinyl chloride concentration of 0.02 ppm measured over an 11.0 hour period. In accordance with 0SHA Standard for VCM Exposure, Title 29, Chapter XVII, Part 1910.93(q) an exposure above 0.5 ppm of VCM requires that I offer to you an opportunity for medical surveillance at the company*s expense if you desire such action. Medical surveillance will consist of a blood testing procedure and a hands on physical examination by a doctor assigned by the company. Results of these examinations will be made available to you or your designee upon your written request and signature of release on the prescribed form. The time spent for blood testing and doctor's examination will not be paid for by the company. If you desire the medical surveillance described above, please contact Mr. Steve Ashby at 491-5039 and he will set up your appointments for the testing and physical examination. Remember, it is not mandatory on your part to accept the medical surveillance. If you do not want to participate in the medical surveillance described above, please sign the attached waiver form and return it to Mr. Ashby.
