Collier / Osage Happy Hollow Lease (CWA) Inspection Date 07/30/2025 Section I - INTRODUCTION PURPOSE OF THE INSPECTION EPA Region 6 Inspector, Matthew Allen, arrived at the H.R. Collier - Osage Happy Hollow Lease (EPA ID No. OKU002220) at 10:30 PM on 07/30/2025 for a site inspection in response to a spill report (NRC# 1438334). The inspection was conducted by the authority of the Clean Water Act (CWA) to determine compliance with Environmental Protection Agency (EPA) regulations. Thad Wells, serving as the company representative, was present during the inspection. There was no other federal, state, or local agency representative present at the time of the inspection. FACILITY DESCRIPTION EPA conducted an inspection at the H.R. Collier, Osage Happy Hollow lease where a ruptured poly flowline released a company estimated 5 barrels of crude oil and 1 barrel of saltwater to the land surface. Spill impacts migrated to a nearby unnamed tributary of Little Hominy Creek and travelled generally south, ultimately impacting Little Hominy Creek. Section II - OBSERVATIONS I observed the release point of the spill which was a 2" poly production flowline which ruptured during a startup procedure. See photo No. 1. For additional GPS location information and photo metadata, please see Appendix 1 (photo log) attached to this report. I observed the point of entry (POE) into the unnamed tributary of Little Hominy Creek. At the time of the inspection the tributary was dry at the POE. See photo No. 2. Approximately mile downstream from the POE into the unnamed tributary, I observed a TDS field reading of 2,999 mg/L at the first pool of water available for observance. This was the highest observed TDS field reading of the inspection. At this same location, a small amount of oil was floating on the surface of the water. See photo No. 3 and 4. Cleanup was underway at the time of the inspection. Industrial absorbent material, absorbent pads and absorbent booms were being used to remove oil from the impacted areas. Continuing approximately 800 feet downstream, near the confluence of the unnamed tributary and Little Hominy Creek, I observed a TDS field reading of 2,543 mg/L. A small amount of oil was continuing to migrate down the unnamed tributary at the time of the inspection. See photos No. 5 and 6. Immediately upstream from the confluence of the impacted tributary and Little Hominy Creek, I observed a TDS field reading of 527 mg/L. This reading was observed in an area which was not impacted 2 H.R. At the point of entry (POE) into Little Hominy Creek, I observed a TDS field reading of 504 mg/L. See photo No. 8. Approximately 150 feet downstream from the POE, I observed oil on Little Hominy Creek and a floating containment boom which was placed by the operator to prevent downstream migration of spill impacts. At the time of the inspection, the boom was properly installed and was preventing downstream migration of the oil. This was the end of the observed spill impacts. Section III - AREAS OF CONCERN The primary area of concern was the oil present along the impacted tributary and Little Hominy Creek. Additional areas of concern included the elevated TDS measurements observed along the impacted tributary and the spill impacts to soil and vegetation along the overland portion of the spill path. Section IV - FOLLOW UP EPA conducted a follow up inspection on 8/4/2025. EPA inspector will conduct follow-up calls, field meetings and inspections to ensure the impacted area is returned to "pre-spill" condition.
