Several months ago the Loss Prevention & Safety Department distributed Survey Inspection Report forms to assist supervisors in conducting an on-going inspec tion program in the plant. A copy of the first five pages of this form is attached just as a memory refresher and as an example of one means of conducting an orderly self-inspection.
