Section I - INTRODUCTION PURPOSE OF THE INSPECTION EPA Region 6 inspectors Kerri McCabe (lead) and Uma Lad arrived at the Arkansas Production Services, LLC / Avera-Effie Johnson Tank Battery ("facility") located from State Highway 7, straight on Hayes City Road (Ouachita CR 68), right on an oilfield lease road at 33.367833, -92.634733 in Louann (Joyce Community), Ouachita County, Arkansas; at 1220 on 12/03/2025 for an unannounced inspection. No facility representatives were present at the time of the inspection. The inspection was conducted by the authority of the Clean Water Act (CWA) to determine compliance with Environmental Protection Agency (EPA) regulations. There were no other federal, state, or local agency representatives present at the time of the inspection. FACILITY DESCRIPTION EPA conducted an inspection on the Arkansas Production Services, LLC / Avera-Effie Johnson Tank Battery facility, which contains five (5) tanks for crude oil and saltwater separation and storage, one (1) active production well, and one (1) inactive production well.

Section II - OBSERVATIONS
I observed the facility signage and an overview of the facility; see Appendix 1 (Photographs Log), Photos No. 1 and No. 2. For additional GPS location and photo metadata, see Appendix 1. I observed the front side of the tank battery, see Photo No. 3. I observed the transfer/product line and catchment bucket with residual oil outside containment, see Photo No. 4. I observed the right side of the tank battery with the sump area in the background, see Photo No. 5. I observed the active production well onsite, elevated due to flooding potential, see Photo No. 6. I observed the drainpipe for secondary containment with a closed valve and cap, see Photo No. 7. I observed the backside of the tank battery with the sump area in the foreground, see Photo No. 8. I observed a low area in the containment berm on the backside of the berm, see Photo No. 9. I observed the left side of the tank battery with an adjacent inactive production wellhead and concrete pad, see Photo No. 10. I observed an active leak from overhead piping at the tank battery, see Photos No. 11 and No. 12. Section III - AREAS OF CONCERN
No closing conference was held due to no personnel with Arkansas Production Services, LLC present during the inspection. Areas of concern at the time of the inspection include the following:
Loose crude oil outside containment at the transfer/production catchment bucket (Photo No. 4), A low area in the berm (Photos No. 8 and No. 9), and An active leak of crude oil from overhead piping at the tank battery (Photos No. 11 and No. 12). Section IV - FOLLOW UP
Inspector will conduct follow-up phone calls with the company representative(s) over the coming weeks and months to ensure areas of concern are properly addressed.
