Re: Compliance Inspection Permit No. 39fc*12--02 An annual solid waste compliance inspection was conducted at your facility on December 7-9, 13, and 14, 1933 by Mr. Tim Chaney of the District 5 Office, with an additional inspection conducted on March 13, 1984. The enclosure to this letter is a summary of the results of these inspections and it will serve as notification to Chemical Waste Management, tnc. of violations of Permit 39012-02, Permit WEW-168, and 31 Texas Administrative Code 335. A meeting is tentatively being scheduled for Monday, April 30 1934 at 10:00 a.m. in the Central Office of the Department to discuss the resolution of these violations.
