Type and Purpose of Inspection The purpose of the inspection is to view the activities occurring at the site for their applicability and compliance with the 2017 EPA Construction General Permit (CGP). IV. Nothing in this report affects EPA's authorities under federal statutes and regulations to pursue further investigation or action. V. Facility Description Shay Lane is a nine lot subdivison undergoing residential development in North Reading, Massachusetts that is located off Park Street (the " Site "). The Site which comprises approximately 10 acres is located approximately 0.25 miles north of the Ipswich river. At the time of the inspection, there were three homes within the subdivision that appeared completed and there were several others at different stages of construction. The subdivision also includes several detention ponds used to collect stormwater runoff from the Site. VI. Inspection On December 3, 2020 via a phone call, Stephanie Tougas announced the inspection to Ms. Tracy Adley of Ranger Development Corp., who informed David Murray, the President of Ranger Development Corp. (builder and developer of the Site) of our intent to inspect. Ms. Tracy Adley then confirmed with Ms. Tougas that access was granted by Mr. Murray. Ms. Tougas confirmed the inspection again on the morning of December 4, 2020 by contacting Mr. Murray via email. EPA inspector Neil Handler and Stephanie Tougas (" EPA Inspectors ") arrived at the Site at approximately 11:30 AM. Mr. Murray did not meet EPA staff at the Site for the inspection. EPA Inspectors began the inspection at the entrance to the construction site and began walking the perimeter moving clockwise. At the intersection of Park Street and Shay Lane, EPA inspectors observed silt fences along the western side of the subdivision entrance that were falling down and in need of maintenance (IMG_0433). Near the entrance, EPA inspectors observed a catch basin that had leaves surrounding it as well as some sediment buildup (IMG_0434). Also, at the entry way were two catch basins opposite each other that had open porous grates. EPA inspectors observed accumulated sediment below the grate and in the catch basin 2 ED_019088A_00020249-00002 (IMG_0435, IMG_0436). EPA Inspectors also noted soil stockpiles on the west side of the entrance area that had evidence of erosion and did not appear to be stabilized (IMG_0437, IMG_0438). EPA Inspectors then walked north along Shay Lane and observed the first home under construction on the west side of the road (Lot # 1 on the subdivision plans). Some sediment appeared to be tracked out from the driveway area (IMG_0439). Slightly to the north of the first home was an area with evidence of concrete washout (IMG_0440). The discharge to the concrete washout appeared to be confined to within this area. To the west of the concrete washout area beyond a small wooded buffer area there were silt fences installed along the property boundary. However, the silt fences needed maintenance (IMG_0441). EPA Inspectors continued walking the perimeter, and directly to the north of Lot # 1 EPA inspectors observed a staging area where construction equipment and debris were being stored. Within this area EPA inspectors also noted several 5-gallon containers that were approximately three - quarters full of hydraulic oil. The containers were being stored on the ground without any type of spill containment (IMG_0442, IMG_0443). Proceeding further north EPA Inspectors observed multiple detention ponds at the north west corner of the construction site (IMG_0444) behind Lot # 3 (also the location of # 5 Shay Lane which was one of the constructed houses). The Outlet structure and gravel spillway for the final detention pond lead offsite and north of the Site towards Gloria Lane (IMG_0445). There were silt fences along the down slope area, but they were down and in need of repair. EPA Inspectors were unsure which way the discharge from the detention pond flowed on Gloria Lane but EPA inspectors did note a catch basin approximately 1000 feet to the east along Gloria Lane. EPA Inspectors continued walking the perimeter and came to another detention pond on the north east side of the Site between Lots 4 and 5 on the subdivision plan (IMG_0447, IMG_0448). EPA inspectors observed silt fences around the detention pond area that needed maintenance. EPA inspectors noted the concrete Outlet structure in the detention pond (IMG_0448) and found the outfall pipe for the Outlet structure exiting through the northern wall of the detention basin (IMG_0449). There was flow coming out of the outfall pipe which appeared to flow along the northern boundary of the property in an easterly direction potentially ending up in a pit dug into the ground in the northeast corner of Lot 5 (IMG_0450, IMG_0451). EPA Inspectors continued walking along the perimeter along the east side of the Site, where EPA inspectors noted a slope that had evidence of erosion (IMG_0452). EPA inspectors also noted a small excavated pit with silt fences above it that needed maintenance (IMG_0453). Stormwater runoff along the eastern portion of the Site appeared to be directed to this location. As EPA inspectors continued our inspection along the eastern perimeter of the Site, EPA inspectors noted evidence of sediment leaving the site (IMG_0454). There were silt fences located beyond the eroded areas, but portions of the fence needed maintenance (IMG_0455). 3 ED_019088A_00020249-00003 EPA Inspectors started walking back to the entrance to the development. On the way, EPA inspectors noted a catch basin in front of the house constructed at # 6 Shay Lane (Lot 7) that had leaves and trash on top of it and another catch basin on the opposite side of the street that had accumulated sediment surrounding it (IMG_0456, IMG_0457). Near this location, EPA inspectors noted a street sweeper (IMG_0458). EPA Inspectors then returned to the development entrance and concluded the inspection. There was no out briefing since a site representative was not present during the inspection. EPA Inspectors departed the Site at approximately 12:30 PM. Stephanie Tougas followed up with an email to Mr. Murray indicating that EPA inspectors had completed their inspection and that EPA inspectors would send the inspection report to him soon.
