SAFETY AND HOUSEKEEPING ACTION COMMITTEE April 30, 1976 ACTION REQUEST
Over an extended period of time, you have all been involved in a concerted effort to control the installation and use of ball valves. This action request is intended to be a final followup to implementation of our applicable P-Bulletin (P--013^) to verify completion of actions required by previously committed dates (attached) and to solicit comments regarding the effectiveness of our current procedure.
In reviewing actions taken in your applicable areas, check your current status vs. the five key points of the P-Bulletin, as follows:
1. Instrument manifolds design.
2. Proper use in open-ended hazardous services.
3. Use of lockout devices where necessary.
4. Survey to insure open-ended connections are not plugged where removal of a plug could expose personnel to hazardous conditions.
5. Removal or replacement of ball valves in "low-use" services.
The SHAC committee interprets this bulletin to require ready availability within the unit of lockout devices capable of effectively locking ball valves in the desired position during maintenance. Examples of suitable lockout devices are available in several units in the plant and will be on display outside the Cafeteria about mid-May.
While questionnaires are attached to all copies of this request, I expect one answer from each area containing ball valves to be returned to me. The operating supervisor, or equivalent, will be responsible for completing the status report/questionnaire and will return it to me by May 14.
During the guest inspections scheduled in mid-June, inspectors will be specifically looking for implementation of this procedure during the course of their inspection.
