ELLIOTT, John L. PLAINTIFF'S 9/22/76 Patient in today for ventilatory function studies. Will return in six months for progress x-ray - reminder to be sent. With perfectly normal functions, I see no reason to repeat them for a year. JWM:cj 7/31/78 Weight 173, BP 120/74. Patient says he has felt real well until a few weeks ago when he felt congested in his chest and had a little wheeze. In later discussion this apparently has come and gone several times but has not lasted over about 15 minutes. He is quite active in his work climbing ladders, so forth, without difficulty. Progress chest x-ray is thought to show little or no change in the lung fields and probably l>ttle change in the calcifications although with the first film made today with lesser penetration the calcification particularly in the right chest appeared more prominent. Chest is clear, sinus rhythm. The patient has had some nasal congestion from time to time which he calls "sinus" and I've given him some samples of Chlor-Trimeton 12 mif. Repetabs for that. If he develops any more feeling of congestion in the chest, is to call in so I can examine him promptly, otherwise recheck in one year with reminder to be sent. Patient says he has pulmonary functions each year at Monsanto, to be scheduled fairly soon. I've asked him to request a copy for our files here. 1 JWMrmlmv, 11/13/79 Weight 177, BP 114/82. Patient has had pulmonary function studies at Monsanto but forgot to bring them with him. He continues to be quite active. Progress chest x-ray again shows little change when comparison is made back to the last previous film, that one on 7/31/78 but in going back to 1976, a period of three years, there has been very definite progression in the calcification which is thought to be pleural and going back to 1975 the progression is very obvious. Patient has felt quite well, saying that he's felt so well that he keeps forgetting to make a follow-up appointment in spite of his - reminder. He continues to work regularly and has no symptoms from the standpoint of shortness of breath, cough etc. He has had no problems with work, including climbing. He comments in response to discussion about asbestosis that all of the insulators at Monsanto have stopped smoking. Progress chest x-rays show some further increase in the calcification bilaterally. There is nothing to suggest any neoplastfc involvement of either the pleura or lungs. Ventilatory function studies are slightly better than those recorded at Monsanto 9/26/80. Insofar as FVC is concerned and only slightly better in FEV1. Patient wishes to receive a reminder again to return in a year. JWMrskg LAMO12242 SC 11022
