kb>-uj-dlJL March 22, 1967 MONTHLY ACCIDENT SUMMARY February 1967 We had several serious accidents this month but no accident resulted in a disabling injury. Our total year to date remains as one, as compared to one for a similar period in 1966. Our frequency rate now stands at 0.71 versus 0.77 in 1966. One accident that occurred in the Cleveland office will be reviewed by the Doubtful Gase Committee on March 23. Approximately 2400 pounds of VC1 vented into poly charge area at the Niagara Falls plant. The poly had been cleaned and headed up by a chemical' helper who was being trained in this area. The helper failed to install 2 adjacent reactor manhead studs with nuts and when charge had been heated to 139F, the lead-asbestos gasket blew out causing the charge to vent. All personnel left the building for twenty minutes after activating the building emergency vent fans. This incident will be discussed at the weekly safety meetings. Production personnel will be provided with a thread die to chase the threads as necessary to be sure that the studs are not removed from the vessel pad. A pipefitter at Louisville plant was sprayed with VC1 and caustic while attempting to unplug a pipe line at the VC1 unloading station. Although pipe was open at both ends, there was pressure trapped by solidified caustic. Fortunately, a safety shower was close by and the pipefitter immediately washed off the VC1 and caustic. He received no serious burns. Calvert City was notified of this incident. Violation of lock-out procedure and poly safety check sheet by a poly cleaner and charge operator at the Avon Lake General Chemical Plant was one of the nearest potential fatalities we have encountered for some time. The poly safety check sheet had been filled out by the cleaner and charge operator but cleaner entered the poly without locking out the agitator. The charge operator started the agitator of the poly (38) by mistake, thinking he had started another poly (39). The operator rode on the Pfaudler blades (25 RPM). Fortunately, the area supervisor was close at hand and quickly responded to the poly cart alarm. The disconnect was disengaged and the agitator was stopped. The helper was taken to the hospital and treated for only minor bruises and cuts. Each shift foreman conducted a safety meeting with his people and reviewed the incident and emphasized the importance of following the safety procedures in the plant. DLD/mlr At tachment /Dennie L.
