Crescent Street Flandreau, SD 57028 Facility Representative: EPA Facility ID Number: Reason for Inspection: Inspection Team: 4050001 Routine compliance inspection Marta Grabowski, Inspector, EPA UST Program UST System Description The Flandreau Indian School (Facility) is a federally-owned Facility located on the Flandreau Santee Sioux Reservation in Flandreau, South Dakota. The fuel at this Facility is for emergency generators and is not sold to the public. The Environmental Protection Agency's Facility records indicate the Facility has one, fiberglass reinforced plastic (FRP) underground storage tank (UST) that was installed in August 2010. The tank capacity is 2500 gallons, and it stores diesel fuel for emergency generators. Piping for the tank is a suction system with double-walled flex plastic. Inspection Narrative On May 21, 2025, the EPA UST program personnel conducted an inspection at Flandreau Indian School in Flandreau, South Dakota. The EPA last inspected the Facility on May 2, 2022, and they selected it for routine re-inspection. Ms. Grabowski, the EPA inspector, emailed the Facility representative on April 16, 2025, and left a voice message to schedule the inspection and provide inspection details (see Attachment A). Ms. Grabowski arrived at the Facility at 9:30 a.m. CDT, introduced herself to the Facility operators, and presented inspector credentials. Ms. Grabowski proceeded to conduct a physical inspection of the UST system as well as a review of Facility records. Photographs of UST system components taken during the inspection are in Attachment B. Copies of operation and maintenance records are in Attachment C. 4050001-Flandreau Indian School Page 1 of 3 5/21/2025 UST Inspection Tank Leak Detection Tank leak detection is conducted by a Veeder Root TLS 300C automatic tank gauge (ATG), which performs continuous statistical leak detection (CSLD) with a tank probe as well as interstitial monitoring with a tank interstice sensor. Records of CSLD tests from the ATG were available onsite for the last 12 months, except for January 2025 and March 2025. Additionally, the EPA inspector printed a liquid status from the ATG indicating "normal" status for the interstice sensor at the time of inspection. Annual operability testing for the ATG console and tank sensor was conducted and passed on September 11, 2024. Additionally, ATG annual testing was conducted and passed on June 3, 2025, and documentation was emailed to the EPA inspector. Piping Leak Detection Piping leak detection is not required; however, liquid sensors are present in each sump to monitor for leaks from the double-walled piping. A liquid status printed from the ATG at the time of inspection indicated "normal" status for three sump sensors. Cathodic Protection Cathodic protection (CP) is not required at the Facility. The portion of tanks and piping that are in contact with soil are constructed from non-corrodible materials. Periodic Testing/Inspection of Spill and Overfill Prevention Equipment Current triennial testing records for the spill bucket and overfill prevention device (flapper valve) were not available at the time of inspection. The most recent spill bucket and overfill valve test records available onsite were dated 2021. Documentation was later emailed to the EPA inspector indicating that the spill bucket and overfill prevention device passed testing on June 3, 2025. Periodic Operation & Maintenance (O&M) Walkthrough Inspections Records of monthly and annual walkthrough inspections were available onsite for the previous 12 months. The last annual inspection checklist was dated September 11, 2024. Another annual inspection was conducted and documented on June 3, 2025, and provided to the EPA inspector via email. Operator Training Class A and Class B operator training certificates were available onsite at the time of inspection. All staff who maintain the UST system are A/B trained. Financial Responsibility Because this Facility is federally-owned, financial coverage for the UST system is provided by selfinsurance. 4050001-Flandreau Indian School Page 2 of 3 5/21/2025 UST Inspection Physical Observations The EPA inspector viewed the spill bucket/fill pipe, piping sump, interstitial sensor riser, and the ATG console. The equipment was dry and appeared to be in good condition. The Facility representative explained an existing issue with the field wiring associated with the Veeder Root probe and provided a copy of an invoice from Midwest Petroleum. The invoice indicated the field wires would be replaced during Spring 2025. Inspection Conclusions and Recommendations At the end of the inspection, the EPA inspector filled out an Onsite Deficiency Identification Form (see Attachment D), which represents initial compliance recommendations at the time of the inspection. On June 16, 2025, the Facility representative emailed test results to the EPA inspector as described in the above paragraphs. Prior to departing the Facility, Ms. Grabowski stated that a full inspection report would be forthcoming, which includes the inspection checklist (see Attachment E). Ms. Grabowski provided the Facility representative with the deficiency form and retained a carbon copy for recordkeeping. Inspection Report Attachments A.
