Art ongoing prospective study of the efficacy of an intensive in-hospital program of comprehensive pulmonary rehabilitation was initiated by Barlow Hos pital and the California Oepartnent of Health in October 1976. This report presents some of the initial findings of the study regarding efficacy and prob able cost/benefit outcomes. Determination of the effect of rehabilitation upon acute morbidity episodes and associated costs as measured by hospital days before and after rehabilitation is of major importance in chronic obstructive lung disease rehabilitation program planning. Study findings of an initial cohort of 96 patients with a mean age of 65.2 years and mean FEV^ of 0.67 liters are strongly positive and consistent with major objectives of the pro gram. Acute hospitalization rates post-rehabl!it at ion are markedly decreased. Projected post-hospitalization rate is 44.6% of the prior year period, or an estimated decrement of 23.4 days per year in the first year for patients at severe levels of deficit. Cost/benef1t outcome projections correcting for disease specific mortality have been prepared based upon preliminary data and suggest positive outcomes of sufficient magnitude to be of major fiscal importance for program planning if sustained for longer periods. The poten tial program/fiscal importance of present trend findings when applied to point prevalence estimates of equivalent target populations has been exaauned for California and nationally.
