Barfoot AN-5 The Second Quarter Safety and Housekeeping Inspection for AN-5 was conducted by the Inspection Team of George Pappas, Jake Linkey, Roger Mears and Rick Sharp on April 23, 1985. General Comment: 1. Bypassing interlock system - good procedure that is working. 2. AVIP - totally in compliance. 3. Plectrons - working O.K. 4. Department has several task forces working on improving safety in handling of high hazardous materials. Operations feel knowledgeable about materials that they are handling. South: Safety 1. Scaffolding on west side of E-12 had no toe board. 2. Could find no light on HCN alarm on 21. 3. Homemade wooden ladder on deck below SO2 rotometers on 1st level. 4. Breathing air station on top deck not mounted. 5. Other breathing air stations have no covers. Housekeeping 1. General clutter under 51D1-1. 2. Rope in pipe rack above SO2 building. 3. Scaffolding material and sheet metal tubing laying loose on top of deck. 4. General housekeeping - good. North: Safety 1. Sign on N.E. corner of dept, needs replacing (HCN Warning sign). 2. Cover goggle sign next to PS-25 nearly illegible. c 3. N2 cylinder not chained at N2 rack by 51R1-2. SC 000323 LA/V100 7 520 Second Quarter Safety & Housekeeping Inspection - AN-5 Page Two 4. Portable building (used in shutdown) not tied down. 5. 51P4-6 breathing air line has no handle on valve. 6. Electrical box about 30 feet off ground in northwest corner of department has no cover and tie-ins are exposed. 7. Steam-line next to 51P94-1 needs insulation to be re-installed. 8. Fresh air hose near southwest corner of control hosue needs inspection. 9. Safety shower light near pit (old 24) burned out. 10. Water hose near 51S18-2 needs inspection. Housekeeping: 1. Doors on oil storage/portable pump locker needs to be fixedwired together now. 2. Insulation pads on 51P5 laying on floor - need to be re-installed. 3.
