Tho OSHA representatives, Roberto Sanchez, Safety Engineer and Theodore F. Balut, Industrial Hygienist, visited the plant on the following dates! Tuesday, May 13, 1986 Wednesday, May 14, 1986 Friday, May 16, 1986 Wednesday, May 21, 1986 Friday, May 30, 1986 Wednesday, June 4, 1996 Tuesday, June 17, 1986 Thursday, June 19, 1986 Happenings on these dates are as followci Tuesday May 13 - Mr. Sanchez and Mr. Balut arrived at the plant at approximately S P.M. They responded due to a newscast giving the initial inforeation of the incident. Pedricktown was about to call the OSHA regional office because five employees had been sent to the hospital. They reviewed the facts that were available at that time and viewed the condition of the Maes Rssin building fro* a distance. They then decided to return the following day for additional information and fact finding. Wedneedav. May 14 - A walk-thru of the Mass building was done by Sanchez and Balutjsevmral pictures wsre taken. A sit down meeting wae held and specific information was requested by them. Sme attachment dated 5/16/66, A date wss set for 5/21 to return for employee interviews and other pertinent information that could be of use for their investigation and a data for 5/16 to conduct personnel monitoring and sampling. Friday. May 16 - Sanchez and Balut arrived to conduct personnel monitoring for asbestos during broken transite siding clean up and for VC1 monitoring during maintenance operations. Balut conducted the monitoring and took a few more pictures and Sanchez went over specific informational itmme that he had requested. NGC 14464 Page 2 Wednesday. Hav 21 - Sanchez and Balut conducted employee interviews with threa fiasc Resin personnel, one lead tech and two technicians. These three BFG employees were not present when the incident occurred. OSHA's intent was to determine their knowledge levels of process procedures, emergency training, safety training and environmental understanding. They also requested additional information on process flow diagrams, GC monitoring data, equipment (autoclave) drawings and computer sketches. Friday, Hay 30 - Sanchez and Balut conducted employee interviews with one instrument man who was in the building at the time of the incident, one Sr. Lead Tech from Hass Resin, one Hass Lead Tech who was present at the incident, and one Hass technician who was also present at the time of the incident. Questions were basically the same as the first interviews. 8anchez and Balut were given Information concerning a corrosion study performed by Bet2 on 4/30/86 plus additional information on specific ball valve pressure ratings and ISO pel flange design criteria. Wednesday. June 4 - Balut conducted interviews with the Allied Maintenance Superintendent, and four Allied Maintenance mechanics. Two of the mechanics were present at the time of the Incident and had received minor injuries. Tuesday. June 17 - Balut arrived to check the 08KA 200 logs for the last 5 years. He also checked the first aid cases for the last 5 years and talked to the plant nurse. All was in order and he was satisfied with the recordkeeping. Thursday. June 19 - Sanchez and Balut arrived to conduct the DSHA closing conference. They explained the eonitoring data collected, the engineering design problems that were found, the lack of proper training given to the Hass personnel with respect to understanding the manway closing device design, and failure to use adequate respiratory protection during VC1 maintenance work. The worker was involved in a clean-up operation and was pumping material out from vessel 8100. The employee's exposure level was derived from four samples collected over a 250 minute period. The remaining 230 minutes were considered zero exposure. 17. Area Director HARRY D.
