Provided CBI notification to facility The following information was obtained verbally from Harley Asher and Ed Nix unless otherwise noted. Background: The Suburban Acres property is a Manufactured Home Community (MHC) operated by Jones Estates, LLC. Per the Indiana Department of Environmental Management (IDEM) Drinking Water Viewer, the property has an estimated population of 190 persons. Since January 2020 the facility has had exceedances for total suspended solids (TSS), total ammonia, E. coli, carbonaceous biological oxygen demand (CBOD), and total chlorine residual. A list of effluent exceedances is included in Appendix D. Process Description: The treatment facility is an extended aeration system with a fixed screen, two aeration basins in parallel, a polishing pond and a free chlorine/dechlorination disinfection system. This report does not contain Confidential Business Information (CBI). Staff Interview(s): Harley Asher is the certified operator in charge of the facility, while Ed Nix is responsible for maintenance and performs onsite operations and monitoring when Harley is not present. According to Harley Asher and Ed Nix, daily operations, sampling, and recording are conducted by Ed Nix, who is onsite daily. Ed also collects compliance samples. Harley Asher completes the Discharge Monitoring Reports (DMRs). Harley Asher began as the operator in charge at the site towards the end of 2023. Before Harley Asher, Justin Stouder served as the operator in charge from July or August 2023 until the transition. Ed Nix said he records the electrical runtime gage for the lift station south of N 175 E, and the total flow volume from the flow meter. The volume recorded by Ed Nix is the daily totalizer volume from the Siemens LUT 430 readout. This value is reset each day. Justin Stouder, owner of Services by Stouder, was not present for the field portion of the inspection, but did participated in the closing conference phone call and provided additional information about the June through November 2023 period when the DMRs reported no Page 3 of 9 discharge. He explained that the discharge pump from the polishing pond had been turned off due to the high ammonia levels, and efforts were being made to bring the water into compliance. There was an approximate two-week period during which the effluent pumps were replaced. Justin confirmed that there was no discharge from the effluent pumps during the June through November 2023 time period. This included no discharges in September 2023 despite the reported values on the September 2023 DMR, which Justin Stouder said were a discrepancy. The EPA asked whether any alternative means of wastewater disposal had been used from June to November 2023, or if there were any overflows from the polishing pond. Justin Stouder stated that no alternative disposal methods were used, and there was no indication of wastewater loss from the polishing pond. He also mentioned that he was unaware of any infiltration or integrity tests performed on the polishing pond. FACILITY WALK-THROUGH OCCURRED: Yes Data Collected and Observations: There are two lift stations for the sewer collections system. One lift station is located south of N 175 E. A daily log of the electrical runtime gauge is kept at the list station south of N 175 E. The second is located at the headworks for the sewer treatment plant. A record of the runtime gauge is not kept for the lift station located at the headworks of the sewer treatment plant. The plant has two aeration basins, east and west. At the time of the inspection the east basin had brown foam on the surface (Photos 5, 25, and 26). The clarifier on the east basin had scum accumulating on the surface (photos 6, 7, and 8). The west aeration basin clarifer had flocs flowing up toward the surface and over the weir (Photos 10, 11, 13, 14 and videos 12, 15, and 16). The splitter box at the headworks was prioritizing flows to the east aeration basin, and away from the west basin (Photo 40). The outlets from the aeration basins join upstream of the flow measuring weir shown in photos 17 through 21). At the time of the inspection, the water at the outlet from flow measurement device was discolored and cloudy as shown in Photo 29. The turbidity of the water was approximately 446 NTU. The ultrasonic device, used for flow measurements, was located above a side chamber adjacent to the chamber with the weir. There was floating material in the side chamber (photo 27). At the time of the inspection, the readout from the flow meter was showing 11.2 gal/min. Water from the flow measurement device went into a polishing pond. The south end of the polishing pond was filled with a black solid material as shown in Photos 30 and 31. There was Page 4 of 9 algae near the north end of the pond as shown in Photo 36. An electrical pole and tree stumps were located on the inside of the embankment forming the pond (Photos 36 and 37). Water from the polishing pond overflows by gravity through a chlorinator using free chlorine tablets. The water continues into a lift station which pumps the final discharge to the receiving stream, Tippecanoe River. A bisulfite based liquid dechlorination chemical is added to the lift station with the final discharge pump. Compliance samples are collected from a yard tap located downstream of the effluent A record of the electrical runtime gage is not maintained. Photos and/or Videos: were taken during the inspection. The Totalizer value (RT or Running Total) for June 1, 2023 is 62,257,800 gallons. In the document "Suburban Acres December 2023 Scan 1.pdf", also provided in an email from Harley Asher on April 23, 2025, there are photos of daily logsheets with the totalizer volume for December 25, 2023 as 65,233,200 gallons. This is a total volume of 2,975,400 gallons (or 9.1 acre-feet) over 206 days, which is approximately 14,400 gallons per day. In accordance with 327 IAC 5-2-8(9), the permittee shall at all times maintain in good working order and efficient operate all facilities and systems (and related appurtenances, i.e., equipment used for measuring and determining compliance) for collection and treatment that are: (1) installed or used by the permittee; and (2) necessary for achieving compliance with the terms and conditions of the permit. At the time of the inspection the system had the following conditions: a. The west aeration basin had brown foam on the surface (photos 5, 25 and 26), b. There was scum on the surface of the west aeration basin clarifier (photos 6, 7, and 8) c. The east aeration basin clarifier had flocs moving up toward the surface indicative of high flows or disruptions in the clarifier flow (Photo 14 and videos 15 and 16), d. The flocs in the east aeration basin were visible flowing over effluent weir (photos 10, 11, 14 and video 12), e. Excessive black solids material was present in the southern end of the polishing pond (photos 30 and 31), f. Excessive algae growth was present at the north end of the polishing pond (photo 36), g. An electrical pole and tree stumps were present on the inside edge of the berm surrounding the pond (photos 36 and 37). Page 7 of 9 3. Maintenance of all waste collection, control, treatment, and disposal facilities shall be conducted in a manner that complies with the bypass provisions set forth below. And Part II, B. 2. Bypass of Treatment Facilities, Pursuant to 327 IAC 5-2-8(12): a. Terms as defined in 327 IAC 5-2-8(12)(A): (1) "Bypass" means the intentional diversion of a waste stream from any portion of the treatment facility. The DMRs for June 2023 through December 2023 report no discharge. Justin Stouder confirmed that the flow measurement on the September 2023 DMR was a discrepancy, and that there were no discharges from outfall 001 in September 2023. The flow monitoring device, located on the outlet from the mechanical plant, but upstream of the polishing pond recorded a total volume of 2,975,400 gallons (or 9.1 acre-feet) during the June 1, 2023 through December 25, 2023 period. Justin Stouder confirmed during the closing conference that no water was discharged from Outfall 001, nor were there known transfers or overflows from the polishing pond during this time period. The approximate area of the polishing pond is 11,400 sf, which is less than 0.3 acres. See the map included in Appendix A. The volume of wastewater generated between June and December 2023 would result more than 20 feet of elevation change in the polishing pond. At the time of the inspection the polishing pond did not have sufficient free board to stored more than 9 acre-feet of wastewater. Since the disinfection system is located directly upstream and inside the effluent pump station, any wastewater leaving the polishing pond through means other than the effluent pump would not have received full treatment. Jonathan Moody conducted field monitoring using an In-Situ AquaTROLL 600 (AT600) S/N 1079129. The AT600 was equipped with a temperature/conductivity sensor, pH/ORP sensor, Rugged Dissolved Oxygen (RDO) sensor and a turbidity sensor. Table 1 summarizes the methods used by the AT600 sensors. Table 2 below shows the summary of the monitoring. The AT600 was calibrated on March 27, 2025 at 2:00 PM using a 3-point pH calibration with pH 4, 7, and 10 buffers, and an ORP/conductivity calibration with a 230mV/8,000 uS/cm QuickCal solution. This calibration was performed in the Westlake, Ohio office. The RDO sensor was calibrated using the 100% saturation method on April 2, 2025 The RDO sensor calibration was performed on-site just before starting the field monitoring. The turbidity and temperature sensors both have a factory calibration. Calibration information is documented in the field notebook for this compliance inspection and in the Calibration Report generated by the VuSitu App and appended to the end of this appendix. A sensor verification was conducted on the pH sensor on April 3, 2025 at 11:30 AM. The summary of the verifications are included in Tables 3 and 4. The pH sensor drift was within 0.1 pH SU.
