C. 20009 CMA 046973 FOREWORD Since 1951, member companies of the Manufactur ing Chemists Association have voluntarily cooperated in the Association's safety program by submitting case histories ot accidents or near-accidents occurring in chemical plants tor inclusion in a monthly publication. These have been compiled in three volumes obtainable from the Publications Service at MCA offices, namely: (a) Volume One (1962) -- containing Case Histones Nos. 1-596; (b) Volume Two (1966) -- containing Case Histones Nos. 597-1097; (c) Volume Three (1970) -- containing Case Histories Nos. 10981623. Additionally, the MCA monthly publication. "Accident Case Histories", dated January 1970December 1973 contains Case Histories Nos. 16241988. Because of the voluntary nature of this safety activ ity no statistical inferences should be made as to the frequency of accidents involving a particular chemical or occupation. The index has been compiled and organised to assist readers searching for accident case histories involving specific chemicals, specific pieces of equipment, or specific occupations. To find a case history of a particular nature, look first under the group category most closely associated, e.g.. Chemical, Operation or Occupation, Equip ment. Explosion and Pressure Release, Fire, Static Electricity, Waste Disposal, Corrosion, or Protective Equipment. The same accident case history may appear under several categories. For example accident case history No. 660 appears under the "Chemical" category as 'acetaldehyde", under the "Operation or Occupation" category as "venting"; and under the "Explosion and Pressure Release" category as "acetaldehyde." Many accident case histories, particularly those involving occupations or equipment failure, are not easily identifiable and may appear under various list ings.
