Leak In V-U Reactor Building G-2 reactor was being charged at 07:00, charge was complete at 07:30* The horn on the new M.S.A. Unit came on, but the water did not come on. Charles Holcomb, Lead Operator, David Thompson, Recovery Operator and Bob Morgan, Charge Operator checked the monitors and found where the M.S.A. Unit went off and water was still not on. Charles Holcomb saw the V.C. coming out of G-3 manhole, G-3 was dumped and being cleared. He knew then that the dump valve has to be leaking on one of the G-line reactors. David Thompson set the M.S.A. Unit off manually. Bob Morgan got all of the Utilities out of the building and sent to the break room and Charles Holcomb got all of the contractors out of thei building and sent them to the Contractor's Shop. Charles Holcomb then went downstairs, closed the dump valve on G-3 reactor that was being cleaned,;also closed slurry dump valve and slurry basket and also reactor rinse valve on the dump line at slurry dump valve. Found G-2 dump valve to be the one leaking. After the above mentioned valves were closed, found gaskets leaking between the dump line .and dump valve on G-l and G-5. David Thompson dialed the vapor release number and gave the location of the vapor leak. Guard was also notified. Unable to tighten flanges on the dump line enough to stop the leak, so vented reactor G-2, waited twenty minutes, checked pressure on same, on fifteen p.s.l. Went downstairs and checked the dump line to make sure that the pressure was off same and flushed with water. Waited fifteen minutes, reset M.S.A. Unit, no V.C. detected, sent personnel back to the building, then dialed the ALL CLEAR Ho. and also called the guard. I then told the Lead Operators - Both V-ll and V-12 Leads to make sure that when a reactor was manually entered that the dump line was pressure checked after blank had been changed in the dump line. This will insure us that this problem will not occur again. Also, wrote this in Shift Supervisor's Log Book and in V-ll & V-12 Log Books. a Fresh air mask was not used because the leak was detected before water was cut on and afterward V.C. concentration was very low.
