Gentlemen:
This is to report we hare had another serious accident.
Again, there is some mystery surrounding the case, hut the facts
are briefly theses

Gulf was cleaning a leaded storage tank (no record of
blending) at Cleveland, July l6th. Our safety engineer again went
over tank cleaning instructions with the management four days before
the tank was cleaned. Of the three men that went into the tank in
violation of our regulations, one of them died and the other two
wers seriously ill. The reason we are writing this xs^ not to try to
place the blame on anyone for neglecting to carry out the rules
promulgated by our Medical Department, but to inform you of the
facts that you may take measures to prevent a similar occurrence in
your territory.

I do not believe publicity should be given thjis or any specific reference made to the company or place or number of men in volved. We must take it upon ourselves to supervise personally, by being in attendance, the cleaning of all tanks where there is any question of a blending record, and as many ordinary leaded gasoline storage tanks as -it--W possible.
