I am also attaching a letter from H. W. Smith in Len's Department, outlining a diagram of one possible flow of examination routine. We would like to finalize the agenda with you Monday, prior to the Tuesday meeting with the doctors. I understand, from Martin Spurlock, that there is in existence, a record form agreed upon by Drs. Blalock and Beidleman but that it is not in use. Perhaps we should have this as a starter on the agenda also. We have scheduled our MID (computer) people to be with us Wednesday. By then we hope to have resolved what examination will be made, what records we will keep, and the modes of recall desired. Purpose of Meeting To review as may be necessary the medical requirements for employees who may be exposed to vinyl chloride monomer; to establish a protocal for a continuing health surveillance program for these employees. The protocal must consider the employee from time of employment throughout his working career with the company. The protocal must also be compatable with the company's intent to computerize each employee's medical hIstory. B. I. Medical record form - It is suggested that the forms now being used at Pace and Calvert City be used as a point of departure. Specific minimum examination requirements should be established for a. pre employment or pre assignment b. six (6) months (employees with 10 years or more exposure) c. annual (employees with less than 10 years exposure) d. employees exposed to an emergency 2. Are any special requirements needed because of sex or race? . 3. What values are to be considered normal? 4. Are there any special neurological requirements or problems? C. Coordination between the Consultant and the Contract Physicians. D. Handling of medical information obtained as a result of the medical sur veillance program that may not be pertinent to purpose of the program but may be of concern to the employee. 1.
