Pursuant to your telephone conversation with Dr. Forbes yesterday, enclosed please find copies of the current eligibility lists for the Painesville Plant. The section outlined in red are those employees under 40 years of age.
Eligibility consists of those in Grade 25 and above. Employees in this category who are 40 years of age and over are entitled to an annual exami nation, and those under 40 years are examined biennially. (See enclosed letter of J. H. Wolfsie, M.D. on April 19, 1968.)
Please be advised, also, that the basic periodic health examination provided for these management personnel through Executive Health Exami ners is paid for by Uniroyal.
Necessary changes, if any, should be called to our attention at this time. Throughout the year would you please notify us of any changes in mailing address, transfer to another location, etc.

Executive Periodic Health evaluation Examinations
In order to achieve the main objective of such a program, viz,, the maintenance of health of UNIROYAL's more responsible management personnel, a new revised examination program is being introduced.
All those in grade 25 and above will be eligible. Em ployees in this category who are 40 years of age and over will be entitled to an annual examination, and those under 40 years of age will be examined biennially.
Under the supervision of the Corporate Medical Director, all examinations will be performed through Executive Health Examiners, which organization will directly contact the individual employees for examination appointments.
A copy of each examination report will be sent to the Corporate Medical Director. This is essential to help fulfill the purposes of these examinations, including the determination of adequacy and effectiveness of the program, establishing the necessary rapport with both the examining physician and the employee's per sonal physician so that the employee will derive the most benefit from the examination, and follow-up to assure that abnormalities and deviations from good health practices are being corrected. These records will be handled in a strictly confidential manner in accordance with the Code of Medical Ethics and their contents will not be available to anyone without specific permission from the involved employees.
The examinations will be more comprehensive and of better quality, will include the same content in the field as in New York City, and will include consultation with the examining physician who in each instance will be a qualified internist. Although these examina tions are on a voluntary basis, their value to the individual is obvious and participation should be encouraged.
Those considered for promotion to grade 30 and above will be expected to participate in this program .
