The onset of this man's illness was apparently an acu respiratory infection and prior to this time he had apparently been usually well except for occasional vaue sensations in his chest, above described. It might well be that the weakness and shortness of breath of which he complains are sequential fron ti acute respiratory illness. It may be accentuated by the fact tl he undoubtedly has cardiac abnormality as evidenced by faint bl< murmur indicative of valve failure and also by the definite x-r* evidence of moderately enlarged heart which strongly suggests tl heart usually described as a mitral regurgitant heart. Insuffi< symptoms end physical signs'at present prevents the establishmei of tliis latter diagnosis. Unquestionably, the man has some myo cardial change and unquestionably he does have valvular leak. r. conditions could be responsible for some shortness of breath es pecially during the per> od following an acute respiratory illnes
Briefly then, the noteworthy findings in this patient refer first to his heart, second to his lungs, in the latter wh a minor degree of interstitial fibrosis possibly due to pulmonai asbestesis can not be entirely fuled out. It seems unlikely ths there will be any further progress in the interstitial fibrosis if the man Is removed from the atmosphere of asbestos weaving; and indeed It Is entirely possible that considerable clearing of this may occur but whether there will be cessation of his sympto will depend upon the condition of his heart, 1 believe.

Dear J.'r. Danville:
The following re^'ort is submitted upon the examination of JOHK RESKO. JR.
PERSONAL HISTORY: Age 50; married; wife living and well; two children, oldest 19, youngest 18, both living and well. Family history unimportant. He was born in Bridgeport.
OCCUPATIONAL HISTORY: He began work at American Graphophone on Edison records where he remained for 7 months. He then went to the I.ialleable Iron working as a core maker for 8 months; in the oxidizing plant as core maker for one year. He was in the Army from 1907 until 1919 after which he wait to work at Bridge port Fetal Goods, remaining ther.e about 7 months and then went to Columbia Graphophone Company, pressing records for lg years. He was laid off and then worked for the City for 3 or 4 months. After this he entered the onploy of The Stanley Rolling Mill working for 3 months. Then he vent to Raybestos Mfg. Co. about 1S20. He quit twice and remained out for a short time, but the third time he remained for about 13 years. At Intervals he work ed a month or so at Lighthouse. He quit work about June 30, say ing that he could not stand the work any longer. He had been weaving asbestos while at the Bridgeport plant but when he was tran ferred to Stratford he v;as tracking. He said that he found that tills was much too heavy for him.
PASS ILLNESS; He had Influenza in Panama while in tide Army; Grippe in 1915; no other serious illness, accident or opera tion.
PRESENT ILLNESS: The onset was apparently in February when he v.as out of work about 6 weeks with severe cold and was short of breath with feeling.of heaviness in his chest. Prior to this time he had occasionally been bothered with some shortness of breath and sensation of heaviness in his chest but not severe ly. Since the acute illness he has been weak, has some ary cough in the morning and at night but otherwise during the day he has little or no cough. He is dyspneic on exertion, especially on clir -lng stairs. His legs are weak but this has Improved somewhat since he gave up work. He has a vague feeling of heaviness In his anteri chest and occasional ache over his sternum, and this is sometimes accompanied by a painful sticky sensation within his anterior chest He consulted Doctor Antell a .out three weeks ago after having Doctc Turchek during his acute illness. At the present time he sleeps poorly, his sleep being scmev.hat interrupted by occasional cough. Appetite is finicky. Kis ordinary weight is 200 pounds or better; his present weight is 186.
PHYSICAL EXAMINATION: Ee is a heavy 3 et nan, well developed and nourished, does not appear acutely ill. Eyes: Glasses. Throat: Tonsils either removed or deeply burled. Ears negative. Nose negative. Adenitis negative. Chest: Some flattening of the supraclavicular spaces; not shrunken; no lag; has diminished resonance over both bases; no definite rales; breath sounds clear; expansion good. Heart: P.u.l. In 6th Intercostal space In nipple line; faint blowing systolic murmur at the apex faintly transmitted toward the axilla; rate is slow, 68; occasional dropped beat; snapping aortic second sound. Abdomen is slightly protuberant; no tenderness or masses. Extremities negative; fingers not clubbed. Sputum; specimen could not be obtained for examination.
STEREOSCOPIC X-RAY of the chest reveals faint haziness over the lower half of the ri"ht lung field thereby accentuating the descending bronchial markings somewhat especially In the Inner zone where these markings are sanewhat tortuous and Interlacing. There is, howevsr, no distinct or definite evidence of spider weblike Interstitial fibrotic change typical of asbestosis. On the left side, there is some faint haziness in the mid lung field extending out to the periphery at the anterior level of the 5th and 6th ribs. Again, the spider web like Interstitial fibrosis is not seen. There is some shagginess of the descending truncal markings behind and Just below the cardiac shadow. Theright hilum Is moderatly enlarged and ragged. The left is somewhat ragged but not enlarged. There are multiple fibrocalcific depo sits in and around the left hilum. The lung fields elsewhere are clear except that there Is somehaziness extending up In the mid axillary line on the right side to the clavicle. The heart appears to be definitely enlarged with deviation more pronounced to the left but also ax retracted to the right. Both costophrenic angles are clear. Diaphragms are curved and free.
DISCUSSION: Diagnosis in this patient is indetermi nate. His occupational history x of sanewhat over 13 years as an asbestos weaver suggests the possibility of pulmonary asbesto sis. However, no sputum could be obtained from the patient to determine whether or not asbestos bodies were present. Moreover, x-ray examination of the chest while revealing some haziness over the lower portion of the right lung field does not exhibit typical and characteristic picture of pulmonary asbestosis in the moder ately advanced or advanced stage. The haziness may indicate very .early beginning interstitial fibrosis and it is not believed that this is of sufficient extent to cause any severe disability. It cannot be said definitely that a mild degree of interstitial fi brosis as result of asbestosis does not exist; but certainly such interstitial fibrosis does exist it is not present in any great ly advanced degree.
