at the attached form and find that most of the data included in your attachment is already provided on the COEDS order form. It, therefore, seems this form is simply a duplication of efforts and will not contrib ute to more accurate information.
The policy for dividing any over/under cargo liftings should be divided based on the percentage that each customer has ordered.

Concerning the five questions at the bottom of your loading instruction form, we will notify you in writing and confirm by telephone who will stop the loading, and the necessary inspection and purging instructions.
As for whether the vessel is a UNIGAS ship, or not, I do not understand the reason for this question. Hydroquinone should be added to all liftings by ship and our records indicate this policy has been in effect since the VCM plant was built.

153.15 Requirements for U.S. flag vessel permits.
To have its Certificate of Inspection endorsed with the name of a cargo listed in table I, a U.S. flag vessel must meet the requirements for the cargo in this subpart.
153.16 Requirements for foreign flag vessel permits.
To have its Letter of Compliance en dorsed with the name of a cargo listed In table I, a foreign flag vessel must:
<a) Have an IMCO Certificate, if the flag administration Issues IMCO Certi
ficates, endorsed with the name of the cargo and meet any specific require ments in this subpart that the Com mandant (C--MHMJ may prescribe: or
(b) Meet the requirements of this subpart and 30.0I-5(e> of this chap ter.
