SECTION I - INTRODUCTION
Site Entry and Inspection Objectives
SSO recon inspection for the City of Cahokia Heights.
REGION 5 Lead Inspector, Dean Maraldo, and Inspectors Joan Rogers and Ted Flatebo arrived at the City of Cahokia Heights' (the "City") engineering consultant's (Hurst-Rosche) office at 5 Bank Square in East St. Louis at 02:30 PM (CT) on 04/05/2022. REGION 5 Lead Inspector presented his credentials to Terry Sudholt (HurstRosche) and informed the group that after the opening meeting we planned to inspect known areas of concern in the City's sanitary sewer system. The table above identifies the attendees that participated in the meeting and field inspection. This report is based on information supplied by City of Cahokia Heights representatives and observations made by the REGION 5 inspectors, including: direct observations made by the REGION 5 Inspectors, photographs taken by REGION 5 inspectors, and verbal or written statements made by information supplied by City of Cahokia Heights representatives during or subsequent to the on-site Inspection.
After opening introductions, I asked the group for a sewer funding update. Jim Nold (Hurst-Rosche) mentioned that the City has been working with congressional representatives regarding assistance with sewer infrastructure issues, and specifically with funding support for sewer trunk line and pump station projects and projects that will help separate the sewer collection system from the East St. Louis system. The City representatives discussed potential plans for a 3.5 mile long sanitary trunk lining (Jerome Lane) for a critical portion of the City's collection system.
I asked the City representatives about status of the ongoing SSO discharge at 82nd street. Dennis Traiteur said he believes the SSO discharge at N82nd Street was still attributed to surcharging of the East St. Louis collection system.
Prior to concluding the opening conference, I asked about the City's MS4 permit status. The City representatives did not know if the City was currently covered under any MS4 permit, and said they would look into it.
At this point we concluded the opening conference and discussed the afternoon's plan to observe several areas of concern in the sanitary sewer collection system. The City representatives chose not to join us for the field observations.
After concluding the opening conference, we met with the U.S. Army Corp of Engineers representatives to discuss the plan for observing SSO areas of concern, and departed for the first location, the point where the

I observed the last manhole prior to flow entering the East St. Louis system in eastern portion of the Cahokia Heights system. I did not hear any sewage flowing in the covered manhole.

I observed the location of the chronic SSO point in front of a residential property on the north side of N82nd Street. The SSO was flowing (see Photo 1) and toilet paper debris was observed around the flowing cleanout.

I observed the lift station in the vicinity of Lauralee Drive and Violet. The lift station was in operation and not discharging at the time of the inspection. This lift station was observed discharging sanitary sewage during dry weather in January 2021.

I observed collapsed ground adjacent to the former Village of Cahokia Lift Station 5 (rear), revealing what appeared to be a large storm sewer main adjacent to the lift station (see Photo 2). No evidence of discharge was observed.

After the observations on April 5, 2022, we departed Cahokia Heights and conducted an inspection at another facility on April 6-7. We returned to Cahokia Heights on April 8 to observe the N82nd Street SSO location. The SSO was flowing (see Photo 3) and toilet paper debris was observed around the flowing cleanout. We then concluded the recon inspection and departed Cahokia Heights.

On April 5, 2022, I observed the location of the chronic SSO point in front of a residential property on the north side of N82nd Street. The SSO was flowing (see Photo 1) and toilet paper debris was observed around the flowing cleanout.

SECTION VI - CLOSING CONFERENCE AND FOLLOW UP
Closing Conference
On April 8, 2022, we concluded the recon SSO inspection at the N82nd Street SSO location and departed Cahokia Heights. At 8:55 AM (CT) I conducted a closing conference via telephone with City representative Dennis Traiteur. I asked Dennis Traiteur about the exposed pipe adjacent to the former Village of Cahokia Lift Station 5 (rear). He was not aware of the exposed pipe and said he would look into it right away and get back to me with an update. We concluded the call at 9:03 AM (CT) on April 8, 2022.

Regarding the exposed pipe adjacent to the former Village of Cahokia Lift Station 5 (rear), Sharlin Pfeffer, the City's Assistant Director for Water and Sewer, provided the following update via electronic mail on April 8, 2022.
"Per the conversation you had with Dennis this morning in regards to the hole near the (Jerome & Mousette) lift station; this is a storm sewer drain that needs to be repaired. Storm drains are the Street Departments jurisdiction and they are aware of the needed repair."
