The inspection was (check one) unannounced announced and consisted of interviewing facility representatives, recording field observations, and taking photographs to document site conditions throughout the facility at the time of the inspection. Opening Conference 1) Brief narrative documenting those present, introductions, and explanation of the purpose of the inspection. Attendees: Andi Hodaj and Matthew Schulte (collectively, "EPA") first visited the site at 11:47 am CST and talked to a person that was washing a truck on the property. EPA presented the inspector credentials and asked the person to identify himself. The person refused to identify himself, but said that there was no manager on-site. Minutes later, EPA talked to another person that was driving a pick-up truck onto the site. The inspection team asked him if there was a manager on-site. The person identified himself as Gonzalo Sentemos, but did not confirm his role at the property. He provided EPA with the name of the Top Notch Express owner, Hugo Amaezcua, and said that the owner would not be on-site for another 2 hours. EPA left the site and returned at 1:02 pm CST. EPA talked to a third person on-site that gave EPA a phone number for Mr. Amaezcua, but did not provide his name. EPA called Mr. Amaezcua and explained the purpose of our visit. Mr. Amaezcua stated that he could not be on-site that day, but he gave EPA permission to conduct a walkthrough of the facility and take photographs with the other men present at the property. 2) Credentials presented to: Gonzalo Sentomos and unidentified person at the facility 3) Confidential Business Information (CBI) reviewed: yes no 4) Facility has been individually notified by Permit authority or EPA that it is subject to stormwater permitting requirements? Include operator's description and note any documentation that further establishes SIC code (permit applications, reports, business registries, website, etc). Top Notch Express, Inc. is a trucking company and rents the parking lot at 97 Patterson Road, Joliet, IL, from someone else. Mr. Amaezcua could not identify the owner of the parking lot, though said that other trucking companies use the lot too. Other industrial facilities owned/operated by same business entity? Yes No Describe: Unknown at time of the inspection SECTION II - OBSERVATIONS Pollutant Sources SITE EVALUATION Note location (including whether occurring indoor or outdoor, and whether outdoor areas are paved or unpaved), quantity/size, design issues, any operation and maintenance (O&M) deficiencies (including the nature and extent), potential pollutants, and evidence of exposure to stormwater. Are best management practices (BMPs) in place to minimize or eliminate stormwater discharges from industrial activities? If so, please describe. Storage Storage of Industrial Materials or Products EPA observed oil sheens and fuel in runoff flowing towards Hickory Creek. There were several fuel oil tanks on site without secondary containment. EPA observed several liquid Liquid Storage (e.g., Tanks, Liquid storage drums piled on the banks of Hickory Creek. Storage Drums) Pollutant Sources Waste Storage/Disposal Areas (solid and/or hazardous) Note location (including whether occurring indoor or outdoor, and whether outdoor areas are paved or unpaved), quantity/size, design issues, any O&M deficiencies (including the nature and extent), potential pollutants, and evidence of exposure to stormwater. If so, please describe. 3 Top Notch Express May 3, 2022 Waste Treatment Facilities (e.g., Pretreatment Systems) Fueling Stations/Equipment Maintenance Areas & Cleaning Areas There were fueling stations on-site, including oil tanks, and truck washing taking place during the time of the inspection. Sediment & Erosion Controls EPA did not observe any sediment and erosion controls in place at the time of the inspection. EPA observed discharge of runoff from the property into Hickory Creek. There was visible oil sheen in the discharge flowing down the banks of Hickory Creek. 4 Note evidence of any nonstormwater discharges, including process water discharges, from the Facility, and for any such discharge, describe its nature, source, discharge location, Receiving Water/MS4, Receiving Water appearance, and discharge authorization, if any.
